How to apply for transcript:

(i) Fill the on-line request.


(ii) Make the payment of the applicable fee on-line through payment gateway
using debit/credit cards.
(iii) The fee for issuance of transcript is Rs. 500/- per set. For transcript is to be
sent directly by the Institute to the Foreign University/ies / Management
Institution/s / outside India through DHL Courier, an additional amount of
Rs. 2100/- towards courier charges is to be paid. GST is applicable on the
total amount.
(iv) After submitting your request online, upload the following self-attested
documents along with the payment application at the address given below:

Self attested copies of:

1. Marksheets of Entrance / Foundation / PE I / CPT (as applicable).


2. Marksheet/s of PE II / PCE / IPCE / Intermediate (both front and reverse side,
if subjects are mentioned on the reverse side)
3. Accounting Technician Certificate (ATC), issued at Intermediate level
Examination, if applicable (Accounting Technician Certificate details will be
included in the Transcript if applicant desires and sends a self-attested copy of
the ATC and only to those who have not yet passed the Final Examination).
4. Marksheet/s of Final Examination (both front and reverse side, if subjects are
mentioned on the reverse side).
5. Pass certificates of all the Examinations.
6. Self attested copies of Rank Certificate issued, if any.
7. If Member is qualified any Post Qualification Courses like ISA-AT, IRM, INTT-
AT etc. and wish to include in the Transcript, please submit self-attested
Marksheet and Pass certificate of the same.
8. Self attested copy of Associate Membership Certificate / Fellow Membership
Certificate.
9. Prescribed Academic Request Form or any other Form duly filled in by the
candidate, received from Foreign University/ies / Management Institution/s, if
any to be sent alongwith transcript.

Please note that If applicant opted “Yes” to include the negative attempts in the
transcript, all failed attempts will be included in the transcript. If opted “No” only
passed attempts will be included.. However, if appeared for both groups in an
attempt, and passed in one group and failed in other, both groups attempts will
reflect in the transcript.
If opted “Yes” to include the negative attempts in the transcript, please send all
the Marksheets (Positive/Pass Attempts + Negative/Fail Attempts) along with the
documents mentioned above to process the desired transcript.
In case membership certificate is also required to be sent along with the transcript,
please furnish certified/attested copy of the Membership certificate. For
Certification/Attestation of Membership Certificate, kindly get in touch with M&SS
