The PM lack of responsibility, failure to respond to the scheduled audit, failure to notify
their team, and absence from the office during the audit all point to the organization's
negative calture.
Employee complaints regarding the organization's poor condition, as brought to the
attention of the HSM by Worker C, reveal a negative organizational culture.
The PM showed no apparent commitment to H&S by ignoring worker complaints and
doing nothing to address the issues raised by worker C.
The organization's risk assessment has been outdated for 3 years; this shows the
management's lack of interest in H&S and indicates a negative culture.
The workplace was untidy, and no housekeeping was done. Slips and trip hazards,
highlighted by Worker C, were present, also being negative indicators.
During the audit, supervisor B pointed out that the organization has a high rate of
sickness and illness as a result of injuries and workers getting off work.
Workers were using the emergency exit doors for moving chemicals and also blocking
them by storing the chemical drums in front of the emergency door.
Equipment was also poorly maintained in the organization, as witnessed in the scenario;
the mixer got corrosion, and its routine maintenance records were also outdated.
The number of accidents and near misses statistics that were recorded were high in the
organization, as witnessed during the adit; this indicates the negative culture of the
organization.
Accidents occur, but not every accident or near miss was recorded in the accident book.
As highlighted by supervisor B, the worker who got a back injury was not recorded in the
accident book.
In the organization, the recorded accidents and near misses were not reported to the
regulator, which shows the lack of concern of the management about H&S.
No accidents or near-misses investigations took place, so no root causes were
determined and no corrective actions were taken to determine the negative culture.
