of information about the
patient's condition, care
plan, and any recent
changes. This continuity of
care is essential for
providing consistent and
effective treatment.
4:00pm Afternoon Rounds offer an opportunity to
evaluate the effectiveness
of current treatments and
make necessary
adjustments to the care
plan.
