EDUCATOR’S FILE /S ( Teacher Planning File & SBAM File )


1.1. TEACHER PLANNING FILE

Per Grade :
4. Subject Improvement Plan (SIP)
It is based on previous records and or experience i.e. Learner
Performance Analysis for previous year, classroom practice and results
of assessments (Mark Sheets/ schedules). You need to set the TARGET
of the subject for the current year, improving on the previous pass %.
This plan needs to be in place at the very beginning of the year. You
thus need to review and update it QUARTERLY, after assessments that
have revealed particular problem areas (refer to the Item Analysis /
Diagnostic Analysis in your Learner Performance Analysis for each
Assessment Task). Diagnostic Reports for NSC Examination ,focusing
mainly on Physical Sciences and Life Sciences common errors that
relate to your grade, must be used as well. TIMSS Diagnostic Reports
should be considered as well
5. CAPS Document
6. Annual Teaching Plan ( ATP) and or
Pacesetter (for administration of dates purposes – Indicate the
‘Date completed when and as you finish the Topic)
7. Assessment Programme / Programme of
Assessment (PoA)
A separate programme must be done for each subject and submitted to
the school for inclusion in the school assessment programme. The
school might have to change some of your dates. Verify and align this
with Data capturer of SASAMS at school as well.Indicate planned dates
in all the Tasks
8. Lesson Plans
Daily lesson plans / preparations must show how lessons will proceed
within an hour period. This should be a detailed plan and learner-
centred. Science Process skills, Cognitive levels, Specific Aims and
Suggested Activities column in the CAPS document can greatly assist on
what to appear as LEARNER ACTIVITIES in the lesson plan. Teacher
Reflection should be based on the objectives of the lesson in terms of
Science Process skills, Cognitive levels, Specific Aims and Suggested
Activities column in the CAPS and in the ATP.

9. Control Sheet/s by DH / SMT


Evidence, in the form of Control sheet should be available in the
Teacher Planning File. NB: Stamping and signing is not the
complete evidence of controlling teachers’ work .
1.2. School Based Assessment and Moderation (SBAM) File

Outside Cover page for Teacher’s file should display the following
of the information:
File Name of the District ; Name of the School ; EMIS No. of the
School ; Subject ; Grade; Subject Teacher ; Contact details

(Copy and paste your Grade’s ATP )
4. Programme of Assessment (PoA)
5. Assessment Tasks
NB: ALL original Formal Assessment Tasks for all the Terms as per PoA .
Proof of pre-moderation should be visible for each task. Moderation tool
should be evident. Weighting grids/Framework for Tests and Controlled
Tests , showing content/topics coverage , allocation of marks and cognitive
levels coverage should be evident and accompany each test/controlled test
NB: You can file tasks as they are prepared – not all in advance!
6. Marking Tools:
Memoranda / Rubrics / Checklists/ Marking Guidelines
Proof of pre-moderation should be visible for each Marking Tool
7. Moderation Tools
NB: In-school and District/ Cluster Moderation Tools for each Task should be
evident in the SBAM file. 10% of scripts should be moderated by DH on-site
NB: Stamping and signing the Task is not Moderation
8. Recording /Mark Sheets
This list should indicate which answer scripts(if applicable) were
moderated
9. Learner Performance Analysis
(including Item Analysis & Remedial Plan)
There must be some evidence that you have thought about the meaning of the
results e.g. a line graph of results, or written comments on questions poorly
answered should be used. Statistical Data, Distribution of Marks and
Diagnostic or Item Analysis are major components of this item. Mark
Sheet Analysis can also be done using a sample.
Remedial Plan
This is informed by classroom practice, and results of assessments. You thus
need to review and update it after performance Analysis of each Formal
assessment that have revealed particular problem areas.

Should
accompany Learners’ Portfolio
Teacher NB: Learners should be sampled according to their level of achievement in
’s SBAM each Formal Assessment Task. 3 Portfolios for each Task comprising of TOP,
File MIDDLE and BOTTOM achievers.
**Learner Portfolio may have other Tasks but indicate SAMPLED tasks with
colour stickers or any indication.
