CLINICAL HISTORY: 84 yrs male nondiabetic has presented with renal dysfunction (sr
Creat 10.96 mg%). Urine shows trace protein. USG shows 11.3 & 11.4 cm kidney size.

NATURE OF SPECIMEN: Kidney Biopsy.

GROSS MORPHOLOGY: Received 2 vials,


1] Received a linear core measuring 0.8 cm length for LIM in formalin, All embed.
2] Received a linear Core measuring 0.7 cm length for IF in transport media.

MICROSCOPIC DESCRIPTON:
Multiple
PASM show
step serial sections (32) from renal core stained with H &E, PAS, MT and
EIGHT qlomeruli. NONE of them are obsolescent
Glomeruli: They are in the range of normal size. There is no segmental sclerosis
even in the juxta-medullary glomeruli. There is no collapse of tufts or hyperplasia
of podocytes. Capillary lumena are patent diffusely with single contoured basement
membrane. No spikes or pin holes are seen
is no focus of necrotizing lesion or
in basement membranes (Jones! stain). There
fibrinoid degeneration or crescent formation.
Tubules / Interstitium : Tubules show mild degree of acute tubular injury. Many
of the tubules contain neutrophilic casts Interstitium is edematous and show
Scattered foci of neutrophilic infiltrates and lymphocytes. No granuloma is seen.
There is mild degree of tubular atrophy / interstitial fibrosis (occupying less than
25% o£ the cortical area)
Vessels : Interlobular arteries and pre-glomerular arterioles show moderate
degree of tunica media hyperplasia and intimal fibroplasia. There is no focus of
vasculitis or luminal thrombosis.

IMMUNOFLUORESCENCE : FOUR Viable glomeruli are seen. They are negative with panel of
antisera ( gG, IgA, IgM, C3, Clg ). Kappa and Lambda light chain stains do not show
restriction.
