NADU does hereby solemnly affirm and declare
as under: -
That I am a citizen of India
2. That I hereby declare that my name is
mentioned in my few Documents such as
Educational Certificates as AADHAAR CARD and
SMART CARD in few documents.
3. I state that all my names i.e. as AADHAAR
CARD.
SMART CARD and MOHAIDEEN PITCHAI are the names
of one and the same person.
4.That original name of mine is MOHAIDEEN
PITCHAI (Real Name and that there is no fraud
or misrepresentation in making this affidavit
by me.
5. That I am of sound mind and fully competent
to make this application.
I am executing this declaration to be
submitted to the concerned authorities to
record that both are my names.
