HMFC Worship Team Application Form Revised
The purpose of the worship team ministry is to use our instruments, voices, and talents to glorify God
and to use them as a tool to engage people in worship. We play instruments and sing not for us but
always for God's glory, as an example and model for others in the congregation to do the same. As a
member of the worship team ministry, I promise to do the following things:
1. Time Commitment:
a. I commit myself to faithfully practicing in preparation for rehearsals and church worship
times. This means that I need to look at the lead sheets, listen to the music if I can, and
practice my instrument or vocal part.
b. I agree to be on time and know my parts before rehearsal.
c. I will arrive early enough to set up my equipment prior to rehearsal.

ii. Worship Services are: Sunday at 10 am including 9 am prayer.

4. Tithes and Offering
5. Sunday Worship Attendance and practices
6. Bring Bible every Sunday – real Bible and not an app in cp
7. Sunday Worship take down notes
8. Man to Man/Nurturing/Bible Study/Cell group
9. Home worship, IP, and early morning prayer
10. Conceive Person/Evangelize
11. 1 hour prayer every day
12. 1 meal fasting before Sunday
4. Servant Attitude: I commit to developing and demonstrating a servant’s attitude and heart. I
understand that participating on the worship team is an act of privileged service to God and
the church body. I will strive to put others before myself and assist wherever needed (i.e.
stage tear-down and set-up, picking up garbage and water cups from the stage, keeping the
stage clean, etc).
5. Worshipers, not Performers. There are plenty of venues in which musicians perform music,
but this isn’t one of them. The sole purpose of participating on the team is to lead the church
in worship.
6. Be a Peacemaker: The Bible says “Blessed are the peacemakers.” (Matt. 5:9). If I have an
offense with a leader or someone else on the team, I will go to that person in love and
attempt to work it out. If we are unable to work it out I may then approach the worship
leader or a pastor on staff for assistance in resolving the issue.
8. Spiritual Maturity: I understand that serving and leading in worship requires that I have an
ongoing relationship with Jesus Christ. I commit to seek a deeper and relationship with Jesus
Christ as a follower of Him.
9. Testimony outside the church: The Bible calls Christians to be a light in this world of
darkness. As part of the worship team I will commit to interact & behave in a godly way;
including being examples to others in the church through my personal life. I will not be
involved in any behavior that doesn’t reflect a godly life. No smoking, drinking alcohol or
taking ilegal drugs.
10. Dress Etiquette: At Harvester Mission Fellowship Church (Central), we desire the
congregation to “come as you are”. Generally speaking, our style of clothing is casual. Wear
clothing that doesn’t distract from worshipping God.
a. Men: I will wear clothing that isn’t stained, ripped, or otherwise unkempt.
b. Ladies: I will refrain from wearing any revealing or distracting clothing – anything too
tight (i.e. leggings & yoga pants) or bears too much skin. (i.e. no low necklines, no see-
through clothing, and hems of skirts and shorts need to be to the knee, since we are
higher up on stage than the congregation and they look “up” to us. Also if you are a
hand-raiser please make sure your shirt/blouse and pants/skirt meet in the middle and
there is no gap!)
General: Members of the worship team ministry MUST NOT:

- Must have a clean-cut hair. No under-cut, or any trendy cuts design inappropriate as
worship team members.
- Must not wear earrings in any form.
- Must wear clothes with collar during official services.
Must not wear jersey or sando during practices.
1. 1st and 2nd offence may result to a Warning. Sunday worship service absents may be subjected
for warnings.
2. 3rd offence – DA for 1 month
3. Assignment for 1 month – Bible Reading, prayer and fasting, attend practices, bible study, and
all activity of the church must be attended. (New members of the ministry)
4. If under DA, the member cannot use instruments and cannot stand in the pulpit until DA has
been lifted.
Additional Note:
1. Every member of Worship Team ministry must report their daily Bible reading from Mondays
to Saturdays until 12am. If the member was not able to comply, an agreed penalty of 5 pesos
will be incurred and should be given to the ministries assigned treasurer.
2. All impressions for the Bible reading should be posted in our Official FB Group for Worship
team members named: Central Mahanaim Worship Team
3. All worship team members MUST attend prayer-gathering every Saturday (Time: TBA) and
Sunday 9:00AM – 9:30 AM (before worship).
4. Non-compliance to #3 shall result for the member not to stand in the pulpit during that that
same service time.
5. Non-compliance to #3 shall be subjected to penalty as agreed upon by the members
amounting to Php 20.000 only.
I have read and will abide by the Harvester Mission Fellowship Church Worship Team agreement
1. Sign the agreement and complete the application. Hand it to a worship leader at church.
2. A worship team member will contact you for a short interview.
3. If it sounds like you’re a good fit, you will audition with the band.
4. Upon a successful audition, you will complete a 31-day trial period which verifies your commitment
and compatibility.
