CONSENT FORM FOR USE AND DISCLOSURE OF PERSONAL INFORMATION AND DATA

I, ______________________________________________, Employee number


___________________________, currently employed by TransUnion GCC PL (TransUnion LLC has a
majority ownership stake in GCC since early 2023; “Employer”) hereby consent to my Employer and the
following members of the TransUnion Group: TransUnion Human Resources (through Workday), located
in the following countries: USA and India to collect, store, collate, process and distribute all personnel
information and data which I have already disclosed or may in future disclose to my Employer and the
members of the TransUnion Group referred-to in this Consent Form for purposes of or in connection with
my role, function, employment and/or involvement within TransUnion GCC PL.

For purposes of this Consent Form, the term “personnel information” includes, my name, identity
number, age, date of birth, address, gender, nationality, language, information relating to my education or
employment history, data relevant to my position and/or employment such as date of hire, position title,
performance history, compensation and development planning/training information.

I understand that supply of the above-mentioned information is voluntary and my furnishing the
abovementioned information will enable the HR Community of TransUnion Group to analyze, draft and
produce reports relevant with TransUnion Group employee complement. I hereby, give my consent to
gather and maintain my personnel information and data in HRIS system via Workday. I also understand
that if, after I have supplied the information, I may make an application for withdrawal of my consent
given to my Employer for use of my personal information and data for the purposes and in the manner
stated in this Consent Form. I understand that I have a right of access to and correction of the
information/data regarding myself in accordance with the relevant procedures under Workday’s policies.

Further, I have perused the document entitled Consent Memo, which summarizes Privacy and Security
policies relating to how and for what purposes my personal information and data will be utilized via
Workday, I also understand that the security policies may be amended from time to time.

This consent shall be valid for the duration of my term as an employee of TransUnion GCC PL, I
confirm that this consent has been freely given by me and I have not been unduly pressured or coerced
into giving this consent. I have also given this consent in a sober and sound state of mind.
