Housing Program/Tahanan ng Bagong Bayani Housing Program;

3. To deliver, request, sign, and receive on my behalf all necessary documents that may be required by any
government agencies in connection with my above-stated application;

4. To receive and/or acknowledge receipt the original copy of the Transfer Certificate of Title upon full payment
of the account and execution of the necessary documents;

5. To file, process, and follow-up Certificate Authorizing Registration (CAR) at the Bureau of Internal Revenue
and Register of Deeds, Bacolod City; and to pay taxes, fees/charges or assessments, if there’s any;

6. To do such acts and things consistent or necessary for the accomplishment of the purpose of this Authority.

HEREBY GIVING AND GRANTING unto my said attorney-in-fact full power and authority to do and
perform any and all acts as may be requisite or necessary to carry into effect the foregoing authority as fully to all
intents and purposes as WE might or could lawfully do as if we are personally present.
