KNOW ALL MEN BY THESE PRESENTS:
I, _____, of legal age, Filipino, married to _____, and a resident
of _____, with sound and disposing mind and memory, and without
having been coerced, intimidated or unduly influenced by anybody,
has hereby voluntarily executed and proclaimed this instrument as
my LAST WILL AND TESTAMENT, affecting my 1/5 undivided share
over a real property found in, Philippines;
I. That should I finally rest in eternal peace, it is my wish and
desire that interment vigil and burial be made in accordance with
the customs and traditions of my faith, and should this be
impractical or impossible, in accordance with the customs and
traditions of my hometown;
II. That upon the execution of this LAST WILL AND TESTAMENT,
I am the registered owner, by gratuitous title, of a 1/5 undivided
share over a parcel of residential house & lot, including other
improvements standing thereon, situated at ______, covered and
described under TCT No_ of the Registry of Deeds of _, briefly
described as follows:

III. That should the Lord Almighty finally summon this soul
from its earthly abode, it is my wish and desire to bequeath, grant
and devise my 1/5 share or respective portion in the said residential
house & lot, in equal shares unto my children, namely, ______, who
are of legal age, Filipinos, and presently, residents of;
IV. That it is my desire and wish, as I hereby bequeath, grant
and devise that should my children do not have any desire to have
and to hold my said portion as their own, to convey, transfer, and
consolidate their respective undivided interest unto my siblings,
who are similarly registered owners thereof;
V. That for the purpose of rendering this LAST WILL AND
TESTAMENT effective thru the proper proceedings in Court, and to
carry out my wishes, I hereby name and constitute: __________, who
is of legal age, Filipino, single/married and a resident of _____, as my
Executor and/or Administrator of this LAST WILL AND TESTAMENT,
and in the event of his/her incapacity, that who may be appointed
competent by the Court;
VI. That I direct that my nominated Executor and
Administrator be exempt from the filing of the Bond;
VII. That I intend that this LAST WILL AND TESTAMENT
revokes, set aside and/or annul any and all of my other will or
testamentary dispositions that I have made, executed, signed or
published preceding this LAST WILL AND TESTAMENT affecting the
property subject hereof;

VIII. That this LAST WILL AND TESTAMENT is a testamentary
disposition affecting my entire estate in the Philippines;

WE, the undersigned witnesses, do hereby affirm that the
foregoing is the LAST WILL AND TESTAMENT of ______, and we
further certify:
That the above-named Testator executed the same while in
sound and disposing mind and memory; That she signed the same
in our presence at the bottom of the last page and on the left-hand
margin of each and every page; and That we, at her behest, have
signed hereunder and on the left-hand margin of each and every
page, in her presence and in the presence of the Notary Public, this
