My Name is Muhammad Ajmal Shahzad.

I have overall 10 years’ experience in HSE field in Oil and Gas Drilling industry. In my whole career I have only
work with two organizations; My current employer is CCDC and the previous organization where I served is
Pakistan Oil Fields POL.

My prime job role and responsibility includes:


 Ensuring that the company health, safety and environmental standards and policies are maintained by
providing support, advice, inspection, on-site supervision for Permit To Work System “hot, cold, entry into
confined space, energy isolation, etc.”, and all precautions specified in work permits that can be
implemented during the work, and its associated activities.
 I develop Job Safety Analysis “JSA” and up-date current JSA as necessary. Similarly manage SOPs, Tool box
talks, Pre tour and Pre task meetings. Conducted Risk assessments.
I also take part in periodic HSE audits & inspections to identify areas for improvements to rectify unsafe actions
and conditions.
I usually carry out different drills like fire, H2S, Fall from Height, BOP/Kick, with different scenarios and evaluate
performance of emergency response team.
 Also carried out near miss / accident investigation using Comprehensive List of Causes system to identify
their root causes and prevent their reoccurrence.
 Also Review Mast-Check with Assistant driller and derrick man to ensure integrity of the drilling rig.
 Similarly Carry out Weekly, Monthly, quarterly checks on rig equipment.
 Similarly Review the Monthly reports of Maintenance/Calibration of portable Gas Detectors, H2S Sensors,
and Fire Alarm System of both office & Camp.
 Also prepare safety administration works such as annual & monthly HSE reports and participate HSE
presentations as necessary.
 Prepare and manage HSE weekly meetings.
 I also provide direction, training and, where appropriate, supervision to enable our employees to meet their
obligation to work safely and with due consideration for the health and safety of others and for the
environment. Also conduct HSE internal training courses, These courses include but not limited to the
following like:
 Introduction of company Health, Safety & Environment Management System
 Permit to Works and Risk assessment.
 HSE induction for new staff, contractors and visitors
 Environment Protection from different kinds of pollution “noise, heat stress, oil/chemical spills, wastes,
etc”
 Fire detection, protection, fighting includes pre-fire planning.
 I have also demonstrated strong diplomatic and interpersonal skills helping to act as company’s single of
point contact with Client representatives & third party auditors regarding site operations safety.

STOP Card – is always in pocket of each worker/manager. After observation/hazard analysis in any of the ongoing
activity; (1) Stop the job (2) Correct the job (3) Write the Stop card and submitted in Drop box (placed at camp, mess,
meeting room, open area where easily available)
Safe Work Practice Card – it’s part of STOP card – if any worker is performing job safely and competently then
Layout – Map of complete site (Entry, Exit, Camp area, Safe zone, Master point, etc.)

Buddy System for H2S
H2S - 5 PPM (easily detectable by smell),
PEL is 10 PPM (eye burning can be quite– alarm starts – Worker can work for 8 hours),
STEL is 15 PPM (eye burning – alarm starts + Light cannot stop – 15 minutes work + 2 hours break = 1 hour per day work
is allowed)
IDLH – greater than 50 PPM and equal to 100 PPM
H2S LEL = 4.3% (43000 PPM) UEL = 46%
H2S Conditions: Green – Safe < 10 PPM , Yellow – Moderate = 10 to 15 PPM , Red – Dangerous > 15 PPM
H2s concentration: 0.005 – 0.13 PPM (minimum odour limit)
H2S Detectors: (1) Sense of Smell (Nose), (2) Lead Acetate, Coated Strips, (3) Colorimeter Tubes (NIOSH Certified)

If a pulse is present but no respiration, give 1 rescue breath every 5 seconds and recheck pulse every 2 minutes. If there
is no pulse, start the CPR cycle of 30 compressions, 2 breaths.
