I, the undersigned, ANSARI MAQSOOD AHMED ABDUL QADIR, Adult, Resides at


H. No. 387, New Gauripada, opp. Aziz Villa, Near Sahil Hotel, Bhiwandi, Dist. Thane, do
hereby declare on solemn affirmation as under.

That I was the students of Municipal School No. 3, Narli Talab, Bhiwandi, and my
G. R. No. __________.

That I have passed the 4th standard exam of __________________ and have
issued a School Leaving Certificate bearing No. _____________ dated
_______________.

That the above referred original School Leaving Certificate has been lost by me.
The same is not traceable. I am need of Duplicate leaving Certificate for the purpose
Validity of OBC.

That if original Leaving Certificate will trace in future I will return the same to the
School authorities.

That whatever is stated hereinabove is true and correct to the best of my


knowledge and belief.
