AFFIDAVIT OF DAMAGE TO VEHICLE


I, Juan Pedro, of legal age, Filipino, single, and a resident of
______________________-Camarines Sur, Philippines, after having been duly sworn to
by law, do hereby depose and say:
1. I am the holder of a Driver’s License No. issued by the Land Transportation Office
of Inter City and valid until April 13, 2032.

2. I am the authorized driver of a motor vehicle as an equitable owner, registered


under the name of Juana de La Crux, my sister-in-law, more specifically described
as follows:

3. On or about May 30, 2024, I parked the aforementioned car in the shoulder area
of Wakanda City, to visit the farm of my brother.

4. After we took our lunch, I came to check and immediately noticed several
vandalisms, consisting specifically of scratches on the left rear door up to the rear
portion of the aforesaid car. Attached herein as Annexes “A”, "B,” and “C”
respectively, are pictures of the damage to the vehicle;

5. After making a sincere effort to find out more about the occurrence, neighbors
and bystanders stated that they were unaware of any potential witnesses or
perpetrators of the damage to my car while it was parked in the aforementioned
location. I do not rule out the possibility that the damage was caused by another
car because no CCTV footage in the vicinity could identify the culprit.

6. That I am executing this affidavit to attest to the truth of the foregoing facts to
recover insurance proceeds and for other legal purposes it may serve.
