model release contract

The person signing below understands that motion pictures, photos, audio or other audiovisual recordings, still or
moving images were taken of him (the MODEL). As consideration for my commitment as a MODEL, as provided in
this contract, I hereby grant the PRODUCER, his heirs, legal representatives and assigns, and those persons acting
with the power and authorization of the PRODUCER, the right to license, film and create videos, photographs and
other still or moving images, in all kinds of formats that incorporate graphic representations of my image and to use
them in any MEDIA, known or to be known, that is created (the CONTENT). The MODEL also agrees with and
guarantees that it authorizes the following:

a) all intellectual property rights, without restriction, and unlimited permission, without restriction, to use, reuse, publish
and republish photographic portraits, photographs, videos or recordings of the MODEL or in which it may appear
included totally or partially, in composition or distorted as to character or form, without restriction as to changes or
transformations, in conjunction with your own name or fictitious name, or reproduction in color or otherwise, made by
any MEDIUM, known or to be known, for illustration, art , promotion, advertising, trade or any other purpose without
restriction.

b) the MODEL waives any right it may have to examine or approve the finished product or products, or advertising
copy, or printed material that may be used in conjunction with it or whose use may be applicable.

c) releases, discharges liability and agrees to hold harmless the PRODUCER, its heirs, legal representatives or
assigns, and those persons acting under the power and authorization of the PRODUCER, from any liability for
confusion, distortion, alteration, optical illusion or use in the form of composition, whether intentional or not, which may
result from or be produced during the taking of the photograph or in any subsequent processing, as well as any
publication including, without limitation, any claims for defamation or invasion of Privacy.

d) affirms that he is over 18 years of age and has the right to contract in his own name. You also affirm that, before
signing this authorization, release and agreement, you have read it carefully and understand the scope of all its
contents. Through this contract, the MODEL, its heirs, legal representatives and assignees will be bound.

e) THE MODEL grants permission to use its virtual signature to be used in content release permits requested or
required by the MEDIA, known or to be known.

f) THE MODEL agrees to make a video where he confirms that he has been informed about this contract and knows
its content and that he is not being forced, has all his mental abilities at the time of accepting the contract and is not
under the influence of alcohol or hallucinogenic drugs.

Definitions: MODEL means the person photographed, filmed, or recorded and includes his or her appearance, image,
and voice. PRODUCER means a photographer, illustrator, film director or cinematographer, or any other person or
entity that photographs or records the MODEL. MEDIA means any medium including, but not limited to, digital,
electronic, print, television, internet, film, radio, or other media known or invented. CONTENT means any photograph,
film, audio or other recording, with still or moving images, taken from the MODEL.
