This form aims to serve as a guide for personnel conducting key informant interviews (KIIs) with local
government units (LGUs) to assess the socio-economic climate and the existing condition of water
supply and sanitation services. This form also provides a list of initial data/information to be requested
from the LGU prior to the field work, to identify any needs for follow-up during the field visit.
