AFFIDAVIT
I, JUAN DE LA CRUZ, Filipino, of legal age, and a resident of One Two Three, Philippines,
after being sworn to in accordance with law, depose and state:

1. That I am the OWNER, of a certain motor vehicle that is more particularly described as
follows:

Series : NMAX Type of Body : MC
Engine No.: G3E4E-777 Yr. Model: 1999
Certificate of Registration No. : 77777

2. That the original COLOR of the said motor vehicle was " MATTE BLACK ";

3. That due to its fall, the said motor vehicle obtained scratches, so have I have decided to
change its color from " MATTE BLACK " to "RED";

WHEREAS:

4. I JUANO DELA CRUZ, a mechanic and painter, with work station at ANG ANG’s motor
parts and accessories shop, with business address at (address).

5. That I, JUANO DELA CRUZ, was the one who change the color of the above-described
NMAX Yamaha Motorcycle from color “MATTE BLACK” to “RED”;

6. That I vouch to the fact that I did the change of color of the said motor vehicle upon the
instruction of the registered owner JUAN DE LA CRUZ;

7. That the change of COLOR is not intended for any illegal or unlawful purpose but solely due
to the foregoing reasons;

8. That we are executing this Affidavit to attest to the truth of the foregoing and in support of the
application for the change of color of the above-described motor vehicle on its Certificate of
Registration.
