Site Visit Report

Site: Lumbini Provincial Hospital(LPH)


Date: 23-25 Jan, 2025
Time: 9:30-6:30
Purpose of visit:
1. To check the position of shear, wall as per the working drawing.
2. To check the rebar placement and lapping zone according to bar-bending schedule.
3. To observe the location of sump pit and arrangement of rebar on it.
4. How the workability and time elapse of manpower takes per day to perform the work.
5. To provide the instruction related to safety management while performing work in site
without any serious accident.

Observations:
1. Rebar arrangement of raft top and bottom with lapping zone.
2. Lift pit shear wall rebar arrangement.
3. Concreting work for raft.
4. Placing formwork in lift portion for concreting.
5. The way of placing stirrups and binding the bars with binding wires.
6. Excavation and brick soiling over footing area.

Instructions provided:
1. While preparing measurement sheet(bill) each cut bar length should be measured.
2. While binding ties in columns and shear wall the hook of ties should be bend
properly.
3. Each ties connected to bars should group with binding wires tightly.
4. Proper vibration should be done over concreting.
5. The cover should be maintained as per the drawings
6. The concrete slump should be within the range not more than 180mm.
7. Provide the detailed safety management to each and every manpower.
