AFFIDAVIT OF LOSS

depose and state that:

1. I am the owner of a Philippine Passport No.

of Foreign Affairs (DFA) of Cebu. It expired on October 5,
2023;

2. The last time I saw my passport was on August 2024. I lost


my expired Philippine Passport while I was transferring from

3. Despite diligent efforts in searching for it among my


belongings, I am unable to find it to this day;

4. In order for me to acquire a new passport from the DFA, I


need to present an affidavit of loss in lieu of my lost
passport;

veracity of the foregoing statements and to declare that my
Philippine Passport is LOST and I am requesting for another
one to be issued.
