(For New Trade/Renewal of existing Trade License)
for the year 2023-2024

(Issued under Section 521 & 622 of A.P. Municipal Corporation Act, 1994 (Formerly GHMC Act))

ROC No: 00998-2023-QM Date: 24/01/2023

1. Trade License is issued to Sri/Smt P ADHIVISHNU, to carry on the Trade / Operation of M/s Radhika Mobiles
(Non - Eating / Non-medical Establishments,Mobile Phones & Its Accessories including Sim Cards) at premises
No: 113/A , Radhika Mobiles, R CRoad, TPT Vide TIN No: TL/04207/2016 for the year 2023-2024.

2. License Fee Rs: 1500/- is paid on 24/01/2023 for running Renewal of Trade License for the financial year
2023-2024.

The trader shall fulfill the following conditions; otherwise the trade license will be cancelled by the undersigned
without giving any further notice.
1.Exclusive container (Dust-bin) for storing the waste generated within the premises shall be installed and
maintained by the Trader.
2. Street littering is to be avoided by the Trader.
3. Plastic carry bags less than 40 microns thickness shall not be used by the Trader for any purposes.
4. In case of eating establishments, the conditions prescribed under Prevention of Food Adulteration Act,
1954 and Food Safety and Standards Authority of India are to be followed by the Trader scrupulously.

3. This License is deemed to be suspended / cancelled, if the undersigned considers such action necessary on
the grounds of public health, convenience of safety or if any of the restrictions or conditions of license or any
provisions of the Act or those bye-laws are infringed or if the license has been obtained by any fraud or
misrepresentation.
4. This License is invalid if the building structure is un-authorisedly constructed / effecting the road widening or
not fulfilling the provisions of the A.P. Municipal Corporations Act, 1994 (Formerly GHMC Act, 1955) (or) rules
framed thereunder.

5. This license is only for TRADE PURPOSE. It does not confer any power of permission for either temporary or
permanent exemption from the provisions of any other section of the A.P. Municipal Corporation Act, 1994
(Formerly GHMC Act, 1955), bye-laws or rules in force.
