Basic Medication calculation
The basic formula for calculating drug dosages is commonly
used and easy to remember:
D = desired dose (i.e., dose ordered by primary care provider)
H = dose on hand (i.e., dose on label of bottle, vial, ampule)
V = vehicle (i.e., form in which the drug comes, such as tablet
or liquid)
Note : 1 gm = 1000 Mg
Formula = D * V / H = amount to administer
Example 1
•A Client is ordered 50 milligrams of Amitriptyline
(25-milligram) tablets are available. How many
tablets will you give?

Formula = D * V / H = amount to administer

 Result: (50 × 1) = 2 tablets

•A Client is ordered Erythromycin 500 mg .(250
mg in 5 mL are available. How many ml will you
give?
Formula = D * V / H = amount to administer

 Result: (500 × 5) = 10 ml

•A client is ordered Dilantin 50 mg orally .
125mg/5ml of Syrup is available. How many
milliliters will you administer?

A client is ordered 50 micrograms of Fentanyl citrate
intravenously. 0.1milligrams in 1 milliliter of liquid for
IV Injection is available. How many milliliters will you
administer?
Formula = D * V / H = amount to administer
 * convert mg to mcg ....0.1mg × 1000 = 100 mcg

 Result: (50 × 1) = 0.5 ml

 RATE: Volume(ml) = (ml/hr)
Infusion time (hr.)
 volume x drop factor = gtts/min Or drops/min
infusion time in minutes
• Macro drip = volume x 15 / hr x60

• Micro drip = volume x 60 / hr x 60

• Hemoset (blood transfusing) = volume x 10 / hr x 60

• If you have 1000ml Normal saline to infuse over 8
hours, what is the rate per (ml/hr)?.
• RATE: Volume(ml) = (ml/hr)

 Result: (1000) = 125 ml/hr

The physician has ordered IV Saline 1500 milliliters in
12 hours using 15 drops per milliliter
infusion rate. How many drops per minute need to
be delivered?
Macro drip = volume x 15 / hr x60

 Result: (1500 × 15) = 31. 25 drops/min

The provider has ordered 1,000 mL Lactated Ringers to infuse
over 8 hours. You have a macro-drip tubing with a drop factor
of 15 gtts/mL. Calculate how many gtts/min to set as the IV
flow rate.

Macro drip = volume x 15 / hr x60

 Result: (1000 × 15) = 3.125 gtts/min

•Dr. orders your patient to receive 125 ml of D5W
an hour for the next 8 hours. What is the drip
rate per minute using Micro-drip?

Micro drip = volume x 60 / hr x 60

 Result: (125 × 60) = 15. 625 drops/min

•A patient is about to receive one unit of blood
over 3 hours, the blood unit contains 450 ml,
what is the required rate in drops/min?

• Hemoset (blood transfusing) = volume x 10 / hr x 60

 Result: (450 × 10) = 25 drops/min

Dopamine
• Description: a catecholamine neurotransmitter used to
treat hemodynamic imbalances, poor perfusion of vital
organs, low cardiac output, and hypotension.

• Uses: A medication used to treat low blood pressure, a


weak heartbeat, and poor blood flow to the organs.

Indication: For the correction of hemodynamic
imbalances present in the shock syndrome due to myocardial
infarction, trauma, endotoxic septicemia, open-heart surgery,
renal failure, and chronic cardiac decompensation as in
congestive failure

• Give your patient 6mic/kg/min of Dopamine drug (patient
body weight 70kg), We will draw from vial 200.000 mic and Dopamine
dilute it in 50 ml NS then connected with the infusion pump, 200mg….5ml
What is the IV infusion rate in (ml/hr)?

 * convert all the vail from mg to mcg ....200mg × 1000 = 200,000 mcg
1. Calculate the amount of Dopamine that will enter the
patient( as his weight) in one hour
 Formula: Dose (mic/kg/min) × weight (kg) × 60 (minutes)
 Result: 6 × 70 × 60 = 25,200 mic/kg/hr

Dobutamine
• Uses: A medication used to treat a weak heart beat in
patients with heart disease or after heart surgery.

• Description : A beta-1 agonist catecholamine Treats


heart failure by strengthening your heart muscle . It is
proposed as a cardiotonic after myocardial infarction or
open heart surgery.

• Indication : short-term treatment of patients with


cardiac decompensation due to depressed contractility
resulting either from organic heart disease or from cardiac
surgical procedures.

Give your patient 8mic/kg/min of Dobutamine (patient body

 Formula: Dose (mic/kg/min) × weight (kg) × 60 (minutes)

 Result: 8× 75× 60 = 36,200 mic/kg/hr

Heparin
• Uses: Heparin is used to treat and prevent blood
clots caused by certain medical conditions or medical
procedures. It is also used before surgery to reduce the
risk of blood clots.
• Description : Heparin is an anticoagulant (blood thinner)
that prevents the formation of blood clots.
• Indication: prevention and treatment of thrombotic
events such as deep vein thrombosis (DVT) and pulmonary
embolism (PE) as well as atrial fibrillation (AF)
Example 13
• Give your patient 1200 IU/hr by syringe pump. What is the IV infusion
rate in (ml/hr)?
1. prepare the dose for six hours (as medication stability or as doctor Heparin
order): 25,000IU…5ML
5,000IU…1ML
 Result: 1200× 6 = 7,200 IU/hr

2. How many ml do we have to draw from the vial?


5,000IU diluted in ....................................1ml (as vail concentration)
7,200IU ( total dose for six hr)………...???

 Result: ( use Cross Multiplication) = (7,200 × 1) = 1.44 ml
