INTUBATION PT

→ Placing a tube into the body through a natural hollow space


→ Inserting an endotracheal tube into the airway
1. BASIC
• Good Morning My Name Is ….
• Today I would like to perform mask ventilation/intubation
• May I confirm patient’s name/age
• Assistance(if provided)
2. Prepare Items Needed
1. Gloves
2. Mask
3. Ambu Bag (check function)
4. Oropharyngeal tube
5. Lubricant
6. Endotracheal Tube (check the balloon)
- Types : RAE ( Ring , Adair, Elwyn)
- Internal Diameter (ID) : Male 7.5-8 Female: 7.0-7.5
7. Stylet – for rigidity
8. Laryngoscope- Adult: MAC3/4 , Light Bulb tight & On
9. Syringe
10. Suction
11. Stethoscope
3. - Prepare items on your right & ensure ETT is ready with stylet in
- Ensure patient is below your sternum and head is tilted and chin is lift ( sniffing position)
Cervical Injury : Jaw Thrust
- Sedative & Paralytic: wait until not responsive
- Take laryngoscope (Non-dominant hand)
- Butt of blade at hypothenar eminence
- Thumb on handle for control
- Use lifting motion not twist/flex at wrist
- Use other hand to pull cheek to get good visual of tongue
- Place blade to the ride side of mouth
- Sweeping the tongue towards the left
- As bringing the blade to midline
4. • Look for the epiglottis &
• Into the vallecula
• And can see the vocal cords
5. - Insert the ET Tube
- In the vocal cords
- Until the middle of 2 black lines on ETT (4cm beyond vocal cord)
- Remove laryngoscope
- Hold the tube against the teeth against hand
- Remove stylet
- Inflate the balloon
- Connect the ambu bag
- Confirm ET tube in airway
• Condensation in ETT during ventilation
• Use steth to check
→ Presence of breath sounds in axilla
→ Bilateral Apex Lung
→ Bilateral Base Lung
→ Stomach ( no gurgling sound)

Ventilation
→ Refers to any system that allows the replacement of gaseous substances with new gases
→ Lungs swap carbon dioxide for new oxygen containing air.
1. BASIC
• Good Morning My Name Is ….
• Today I would like to perform mask ventilation/intubation
• May I confirm patient’s name/age
• Assistance(if provided)
2. → When unable to intubate,choose to ventilate.
→ Remove ETT
→ Suction to remove any fluid that obstructs the airway (saliva/blood)
→ Insert oropharyngeal tube on opposite side then
→ Slowly push the tongue away bringing the tube to the middle
3. • Put mask on patient
- Measure correct size by: cover mouth and nose but not eyes
• Connect ambu bag
• Start ventilation

4. • Check for entry of airway


5. - Connect the ambu bag
- Confirm ET tube in airway
• Condensation in ETT during ventilation
• Use steth to check
→ Presence of breath sounds in axilla
→ Bilateral Apex Lung
→ Bilateral Base Lung
→ Stomach ( no gurgling sound)
