MUSC 190 Performance Request Form

Please follow these procedures to reserve a performance date for MUSC 190 Recital Hour.
1) This form must be completed, then emailed to: Dr. Brand, Dr. Choi and your studio
professor, no less than 14 days prior to your desired performance date. Please save the
file in Word or as a PDF. Other formats may not open and will be returned, delaying the
reservation of your date.
2) The studio professor will then approve it by forwarding it to Dr. Brand with approval.
3) The approval must be in Dr. Brand’s inbox in order to confirm your performance.
4) Music must be turned in to Dr. Choi 14 days prior to your requested date. You may
deliver music in person, leave it in the bag on her door, or email her a quality scan.
5) You will receive a confirmation email from Dr. Brand and Dr. Choi when your
performance request is approved.
