Fieldwork report outline

1. Title page
 Name of Student, Name of University, Course and Year, Name of Agency, Name
of Agency Supervisor, Name of School Supervisor, Dates of Fieldwork Placement
2. Acknowledgements
3. Abstract
4. Introduction
 The student should place the placement/attachment in context by describing the
organizational structure and functions of the agency. It should include:
 a history of the agency
 an organogram of the agency
 the area/department/section the student was placed/attached to within the
organisation
 the purpose and function of the agency
 Client group(s) served by the agency
5. The technical section
 The objectives of the attachment/placement
 List of the assignments arranged between you and your agency supervisor
 Indication of intended goals of your placement.
Demonstrate whether and how the placement has helped you in;
 In your professional development as a social worker
 In your understanding of the people you were working with ( i.e.
clients/community)
 In relating to your work colleagues
 In your understanding of social work methods and their application
 In your skill in recording
Supervision
 Your use of supervision, how you and your agency supervisor worked together to
meet your learning needs.
 How the University supervision helped you in your learning practice.
Achievements
 Discuss the extent to which you have achieved your stated objectives and goals
(also highlight your own strengths).
Challenges
 Discuss the challenges you encountered during your placement and how you dealt
with them and lessons learnt (also highlight own weaknesses).
Case Study
 The student is required to provide a case study from their placement experience
and demonstrate the use and application of theoretical underpinnings and social
work methods.
Recommendations
 To improve service delivery /functions of the agency or any recommendations to
improve the future conduct of fieldwork placements
Conclusion
 The last section of the report giving prominence to the student’s successes during
the attachment and recapping on the main points of the technical section (s)

Appendices
 Charts, graphs, pictures
 Record of activities the student undertook (attach all weekly reports)
 Any relevant references (manuals, text, the internet etc)

 To be attached at the end of the report (sample below).

Report Presentation Specifications


 In line with the principle of confidentiality, your record and report should not contain
the names of your clients.
 It must be spiral bound
 Be spellchecked with proper usage of grammar
 Be properly paginated (formatting of the document is marked)
 Be written in a technical style
 Have a table of contents, appropriate referencing
 Have appropriate structure
 Be written in the third person
 Be concise : minimum 30 A4 pages (excluding the cover page ,table of contents page,
appendices and any diagrams )
 Times New Roman font ,size 12 ,1.5 line spacing
