• In a browser, type the URL for the software.


• Personal Software Subscriptions:

• On-premise and Hosted Subscriptions:

• Enter your username and password, then click the


button.

• If you forget your password, click the


link.

• On the password recovery page, enter the


email address associated with your account

User Settings

Change User Settings


• Click to open the User Settings.

• To change language, click . Select the language


you want to use.

• Click .

• The next set of section in the user settings menu


are the default options of tools such as the Root
Cause Tree@ views and the Corrective Action
Preferences.
• Click the radio button next to the view of
your choice.

Search
The Search function allows you to define which Investigation, Incident or Audit you would like to see.

• To search, type in the search box and the


Investigations, Incidents and Audits will update
automatically.

Filter
Filters allow you to search your Investigations, Incidents and Audits based on chosen criteria.

• Click the required checkboxes under Phases, Types and


Date Range or type a date range in the From and To
input fields.
• Investigations, Incidents and Audits are filtered
automatically.
• To reset filters to the default their settings,
click .

The Events/ Corrective Actions Toggle allows you to switch from the Events view to the Corrective Actions
view or vice versa.

Creating an Incident with Investigation


• To begin creating an incident, locate the navigation
bar.

• Hover over .

• Scroll to, then click on .


• There is also an option to create an incident
without an investigation.
• Choose for minor events that need only
be recorded.

• This loads the Details page. Here, you are asked to


fill out some basic information.
• Asterisks denote required fields.
• Arrows launch list windows.
• Question marks provide explanatory
information.

• Fill in each field on the Details Page.

• Give the incident an appropriate name.

• Select the date the incident occurred.

• Add the Incident Time if known.

window, select the location where the incident
occurred.

• Repeat the previous instructions to choose the


Classification to which the incident belong.

incident as confidential.

• To make your investigation visible in the TapRooT®

On-premise and Hosted users can share investigations with other members of their team.

• Under the Incident tab, click the Sharing button.

• This launches the Sharing window.

Attachments

Attachments enrich an investigation by providing additional information related to the event. After creating an
incident, you can attach text and image files gathered during evidence collection.

• To attach a file, click the tab.

TapRooT® utilizes two investigation workflows. For low to medium-risk incidents the Basic workflow offers a
simplified process. Users may choose the Basic workflow with or without Equifactor® Mode. For high-risk
incidents that require a full investigation use the Major workflow.

clicking the toggle bar located on the Investigation

around an incident.

• Insert Link Allows users to insert a hyper link into the SnapCharT®. Link requires https:// or

• Change to Event On a condition, changes the shape to an event.

• Causal Factor Identifies a condition or event as a causal factor and generates a RooT Cause
Tree®.
• Add Attachment Allows user to attach attachments directly to a shape.

Shapes
• Events: Use for descriptions of what or who does
what.

• Condition: Use for known information about an


event.

• Incident: Use to represent what is usually the worst


thing that happened.

• Connector: Use when a SnapCharT® is too wide for


one page. The connector shows the beginning and
end of a break.

• Text: Use for additional information that does not


fit the function of a shape, for example, date/time.

Creating a SnapCharT®
• To start creating a chart, click on a shape, hold and
drag to the workspace. Release.
• Before releasing a shape, it is displayed as an
outline to help with placement.

• After placing a shape, you can start typing right


away.

• Continue to place shapes until you have the first


draft of your SnapCharT®.

• Use Grid and Guide to help format the


spacing and placement of each shape.

Connecting Shapes
• To connect shapes, click the blue triangle on the perimeter of your first shape and and drag to your second
shape.

• Notice the difference between connecting Events, Incidents and Conditions.


• Arrows connect events and incidents to show a progression through time.
• Lines connect conditions.

Assumptions, Causal Factors and Safeguards


During evidence collection some events or conditions reported may begin as unproven information. These are
represented on the SnapCharT® as assumptions.

• To change an event or condition to an assumption, click on the shape then click the assumption icon in
the tool bar. This changes the outline of the shape to dotted lines.

• Follow the same process to indicate a Causal Factor or a Safeguard . These are identified by
their respective icons on the SnapCharT®.

SnapCharT® Attachments
Adding an Attachment to a shape allows you to easily associate pictures or documents to their counterparts
in the SnapCharT®.

• To add an attachment to a shape, click on the


shape then click the Add Attachment button .

• In the Attachments window, click the item you


would like to attach.

• Click .

• An Attachment is indicated by the icon appearing


next to your shape.

Deleting Chart Elements

• To remove a shape or a line that is not connected to a Causal Factor, click on the shape or line to highlight
it then press the delete key on your keyboard.

• To delete a shape that is connected to a Causal Factor, you must first remove the Causal Factor.

• Click on the shape, then click the Causal Factor button. The software will display a warning. Click .

• After removing the Causal Factor, click on the shape then press the delete key on your keyboard.

Creating a SnapCap
A SnapCap is an image of the SnapCharT® that is generated using the image capture feature.

• Click the Image Capture button to begin.

• This opens Capture Mode.

• Click on the screen and drag to select an image


area. This highlights the selected space.

• If the image is as desired, click the camera icon


at the bottom of your selection to take the
SnapCap.

• Name the file. Click .

• To close image capture mode without closing the

Optional Techniques
Used to gather information to populate the SnapCharT®.

• Access the Optional Techniques by clicking the


Optional Techniques tab on the left-hand side of
the SnapCharT®.
• There are six Optional Techniques to choose
from: Equifactor®, Change Analysis,
Safeguard, CHAP, Causal Factor and Root
Cause Tree®.

Equifactor® Analysis
Equifactor® is a tool used to troubleshoot common equipment problems.

• To use this technique, click the plus sign next


to Equifactor®.

• Give the Equifactor® analysis a name, number or


identifier.

• Click . This opens the Equifactor® window.

An evaluation technique, focused on changes or differences in a situation or process.

• To begin, click the plus sign next to Change


Analysis.

• This opens the Change Analysis window.

• Enter a name, number or identifier.

• Click .

• In the text box that appears, type a performance


factor.

• Click .

• If you make a mistake, click . Retype


performance factor.

• You can add multiple performance factors. To do


so, click .

• Type additional performance factors.

• Click when completed, to move to


the next stage of the analysis.

• The next window will be titled with your first


performance factor.

• In the proper performance text box, describe the


way that your factor should perform.

• In the incident performance text box, type the way


that your factor performed during the incident.

performance are the same, click to duplicate
the text from proper performance into the incident
performance.

• Click the condition button next to the proper or


incident performance to add the performance to
the Drop Zone as a condition.

• Click the event button next to the proper or

the Drop Zone as an event.

• Click the green arrow to analyze the next


performance factor.

A safeguard analysis looks at the process or system that allows a hazard to reach a target.

• Click the plus icon next to Safeguard Analysis

• Type a Hazard.

• Click the question mark icon to access

• In the Tree view, hover over each option. This will make the tool bar visible. Make your first selection by
clicking Select Item .

• Only one selection needs to be made at the top of the tree. Once you have selected an option, Cross off
the others. Decisions are displayed visually, as shown below.

right. Once you have answered all the required questions
click in the top right of the screen, then close

• The TapRooT® report elements are draggable. Click and hold the desired element. Drag it to the
workplace. Release the element.
• The element will fall where the text icon is placed in the workspace.

• There are two types of elements. Grouped


elements allow users to place all the elements in
one section into the workspace at the same time.
Look for the move cursor to begin.

• Single elements are identified by the default


cursor.

• To place a single elements or a portion of a group


element. Expand the menu by clicking the arrow
icon . Click and hold the desired element and
drag it to the workspace.

• Each factor in the investigation grading tool represents an area of your Investigation completed by an
investigator and can affect the quality of your RCA.

• Choose a weight for each factor based on the level of importance of that component of your
investigation. For example, if identifying all your Causal Factors is more important than ensuring that your
corrective actions are measurable; give a higher weight percentage to number 2 than you do for number
9. Weights must total 100.
