CNS Motor Examination Proforma

1. Inspection
- Muscle bulk: Normal / Wasting / Hypertrophy
- Fasciculations: Present / Absent
- Involuntary movements: Tremors / Chorea / Athetosis / Myoclonus / Dystonia
- Posture of limbs: Normal / Abnormal

2. Tone
- Assessed by passive movement of major joints
- Upper Limbs: Shoulder, Elbow, Wrist
- Lower Limbs: Hip, Knee, Ankle
- Findings:
- Normal / Hypotonia / Hypertonia
- Type of hypertonia: Spasticity / Rigidity

3. Power (Muscle Strength)


Graded using MRC Scale: 0 = no movement, 5 = normal power

Upper Limbs

Shoulder Abduction (C5) L/R ___

Elbow Flexion (C5-C6) L/R ___

Elbow Extension (C7) L/R ___
