I have already checked the completeness of the tools, facilities, materials, supplies also the
occupational health and safety of the assessment center. I already received the test package
from TESDA Representative and the contents are Attendance Sheet, Self-Assessment Guide,
application Fotm Specific Instruction to the candidates, Rating Sheet, Assessor’s Guide,
Assessor’s Script and Competency Assessment Result Summary(CARS).

Let us acknowledge the presence of our AC manager Mr. Ricco Joseph Seville our TESDA representative
Rodalyn Ardales from TESDA provincial office.
You do not have to worry about his presence. He is here to oversee the conduct of competency
assessment and to check if the conduct is in accordance with the prescribed competency assessment
methodology and procedures. He will not assess you.
Before I start, pls. sign in the attendance sheet, as I call your name, bring with you your Admission Slip,
Valid ID and ballpen.

I have here with me your Application Form and Self-assessment guide. This indicates that you know
and ready to take the assessment in Healthcare Services NCII
The purpose of assessment is to determine the level of your competency in HCS NC II with the Basic,
Common and core competencies. The assessment in core competencies such as
1.Prepare and Maintain beds
2. collect and maintain linen stocks at end users location
3. Assist in patient mobility
4. Assist in Transporting patients
5. Assist in bio physical support care of patient
6.Handle waste in heath care environment
While the basic and common competencies will be assessed concurrent by the core. The task to be
done are
1. Bed make up (unoccupied)
2. Transfer of patient from bed to wheelchair vice versa
3. Perform waste segregation in hospital setting
in 1hr/30min.
to determine your knowledge and skills we will use demonstration with oral questiong.
After your assessment, you will receive your CARS which indicates if you are competent or not yet
competent.
If COMPETENT, you will receive a National Certificate (NC) to be sent through your email address
given to the AC Officer, print it and bring your it to the TESDA provincial office for dry seal. But if found NOT
YET COMPETENT, you re-apply for re-assessment within 24 hrs after, as long as you are ready and there is
an assessment schedule. In the event that you are still not yet competent in your second assessment, you
need to have a refresher prior to the reassessment.
Whatever the assessment result, that would be FINAL. But if you are not conferred with it, you have the right to
appeal to our TESDA Representative or to the TESDA PO in written form within 24 hours.
Whatever transpires here in the assessment area it should be kept confidentiality. Result of the
assessment ill only be accessed by the authorized personnel. This is to ensure the integrity of the
assessment process & result.
You would notice we have CCTV, to record the assessment which is through the TESDA PO and Regional
Command Center. But rest assured that the videos are also kept secretly.

During the assessment if you experience any physical discomfort or other difficulties plase do not
hesitate to inform me so tat wew can address them immediately.
If you need take medication or have religious or cultural activities to be observed. Let me know. If you cannot
understand instructions kindly inform me so I can translate it in local dialect.
For your personal needs the lavatory and comfort room are located at the end of the hallway.
You can take your meal at the canteen which is located in the lobby. However during the assessment you not
allowed to leave the assessment center premises.

re-phrase and give examples but not able to answer it.
In your demo, you started with bed making, but when you stripped the linens, I notice that
your linens touches the floor.
You interchange the placement of cotton sheet and rubber sheet.
You were able to transfer your patient safely from wheelchair into bed v.v
In your waste segregation, I have observe that you difficulty in segregating waste into
different categories. Next time you have to do it correctly.
