B. EVALUATION OF CREDENTIALS/RECORDS (TO BE FILLED OUT BY THE REGISTRAR)


Please check if the applicant meets the following guidelines for Latin Honors.

The applicant:
has at least 1.5000 General Weighted Average (GWA) from the computed combined
grades from 1st to 4th year levels;
is not an irregular graduating student;
has no grade lower than 1.8 in a major subject from 1st to 4th year levels;
has no grade lower than 2.0 in a minor subject from 1st to 4th year levels;
_____has no Incomplete (INC) and Dropped (DRP) remarks in any subject from 1st to 4th
year levels;
_____has not been sanctioned of any disciplinary action in the College;
_ has satisfactorily completed the course/program within the prescribed residency
period;
has taken at least 75 % of prescribed subjects/courses of the enrolled degree
program in the College; and
has the General Weighted Average from 1st to 4th years levels of
