WORK INSTRUCTIONS TO BE FOLLOWED BY THE APPLICANT:


1] All information given in the work permit must be accurate.
2] Bring along the approved work permit to the site. No work can be carried out without an approved work permit.
3] Produce work permit if requested by authorized personnel.
4] To acknowledge and return the work permit to KTPC upon work completion.
5] Work Permit will not be provided for a duration of more than 90 days continuously.
6] Work Permit applications to be submitted for approval 3 days before the commencement of work, unless for adhoc or urgent work – permit to be
approved prior commencing work. Only contractors with the approved work permit are allowed to commence the work

TERMS AND CONDITIONS OF WORK PERMIT

Work Permit applications to be submitted for approval 3 days before the commencement of work,
1 unless for adhoc or urgent work – permit to be approved prior commencing work. Only contractors
with the approved work permit are allowed to commence the work
Permission must be obtained from Security Unit to enter the building with an approved Work Permit.
2 All workers shall wear and display the given pass while in the working area.

If the contractor violates any of the terms and conditions of the Work Permit, it shall be automatically
3 cancelled hence works are not allowed to proceed.
Prior permission must be obtained from KTPC Property Maintenance Department before tapping into
4 any base building consumables (electricity, water, air-cond etc.).
All workers shall be closely supervised by the Contractor’s Site Supervisor whose name appears on the
5 Work Permit.
All items including but not limited to structures, equipment, installations and systems are deemed to be
clean and in good working condition prior to commencement of work on the floor where access is
6 permitted. The contractor can be held liable if any item/equipment deemed to be in good working
condition prior to him having access to the floor is found to be faulty after the contractor has accessed
the floor or has commenced work.
The contractor shall take all necessary precautions to avoid any damage or stain to the floor/wall,
equipment, installations, structures, and system of the building during working, delivery, loading and
7 unloading activities. The contractor will be responsible to make good the damages (if any).

Smoking is strictly prohibited in the building. Workers are not allowed to bring in cigarette(s), lighter(s)
8 and/or match sticks. Any worker found smoking shall be removed from the building immediately.
Penalty may be charged per violation.
All high decibel works (e.g. drilling, hacking, knocking etc) and works that produce strong acrid odour
(e.g. varnishing, spray painting, gluing, etc) can only be carried out as follows Mon to Fri: between 7 pm
9 to 7 am; Saturdays, Sundays and Public Holidays: upon approval of Property Maintenance Department.

Any nuisance, disturbance, inconvenience to the building services such as electrical (tripping), rubbish,
10 workers using of passenger lift, etc. as well as item 14 (above) - at Property Maintenance Department
discretion a penalty may be charged per violation.
Penalty may be charged for:- 1) any false alarm; 2) (in the event that they had requested for temporary
11 disablement of the fire alarm system) the contractor fail to inform the Property Maintenance Department
to enable back the fire alarm system upon leaving the worksite.
Loading and/or unloading of material shall be at the contractor’s own arrangement. Use only the service
12 lift (if any) for transporting the same.
Contractors / Suppliers who are involved in delivering goods, raw and other construction materials are
responsible for any damage/cleanliness of the service lift. All materials/goods should be in proper
13 packages. Contractors shall be responsible for the proper protection of service lifts and lift lobbies
(including floors) when transporting equipment/materials.

Please park your vehicle at the designated/allowed parking lot. Penalty may be charged per violation.

TERMS AND CONDITIONS OF WORK PERMIT
NO. SAFETY INITIAL
1 Any violation of the safety regulation shall result in the order to stop work.
Contractors shall be responsible for the safety of their workers and shall not expose the occupants of the building
2 and the public to any health, safety and security risks when carrying out any work.
Contractors shall commence work in the correct and safest manners e.g. right attitude, standard and safe
3 methodology, proper tool/equipment/material/substance, etc.
Contractors shall wear/use complete and proper personal protective equipment (PPE) based on the nature of
4 work. All PPE shall be provided by contractors.
Contractors shall not bring any harmful and dangerous object/equipment into this building unless declared and
5 authorized to do so.
Contractors shall not do any forbidden activities such as but not limited to gambling, fighting, taking drugs or
6 alcohol, and stealing/looting.
Contractors shall comply with ALL acts and regulations related to safety and health, as well as their respective
7 work e.g. Occupational Safety and Health (Amendment) Act 2022,Uniform Building By-Laws 1984 (amendment
2021)., Akta Bekalan Elektrik 1990, etc.
Contractors shall obey and abide by related tenants’ safety and other requirements while being on their premises,
8 if any.
Contractors shall be responsible for their own and the others e.g. occupants, public, etc. regarding occupational
9 safety and health.
Contractors shall practice housekeeping at all times and report any suspicious/dangerous findings immediately
10 to Security Unit personnel.
Contractors shall not block any emergency exit e.g. corridor, fire door, staircases, etc with any object. At the sole
11 discretion of the Security Unit, such object shall be removed without notice.
Contractors shall follow related documents e.g. company’s checklist, manufacturer’s manual, chemical labels,
12 etc as one of the guidelines in commencing work.
Contractors shall ensure that any ‘special work’ is being carried out by a trained or competent person only e.g.
13 chargeman, welder, crane/sky lift driver, etc.
Prior permission must be obtained from Property Maintenance Dept. before any hazardous, toxic, corrosive,
volatile or highly flammable materials, such as but not limited to thinner, turpentine, detergent, acetylene,
14 methane, propane, petrol, kerosene is brought into the building. At the sole discretion of the Property Maintenance
Dept such material if not stored or handled properly shall be removed without notice.
Safety signage, tagging and hoarding/barricade shall be installed to the satisfaction of the Property
15 Maintenance Dept before any work can commence. All working materials on site shall be stored in a safe
manner.
Contractors shall ensure that all chemicals to be brought into this building must be informed in advance, and
have proper labels affixed to the container and relevant certificate provided prior to bringing on site. All
16 chemicals have to be properly and safely managed from the point of generation to the point of final disposal
(handling, storing and disposing of).
Contractors shall ensure that all chemicals have to be handled in accordance with legal requirements and good
17 practices. No one is allowed to remove or tamper with (deface, modify, alter) the labeling of hazardous
chemicals.
Except under fire emergency situations, contractors are prohibited to change the status of any firefighting
18 equipment and their associated parts or use the fire-fighting system for other purposes.
If you hear the alarm, do not panic. Leave your items and calmly leave the building through the nearest stairwell,
19 and go to the assembly point. Do not go anywhere else and wait for further instructions.

All Vendors/Contractors are required to monitor and ensure their employees practice personal hygienic measures
20 and to provide disposable masks and gloves which should be always worn. Any worker found sick must be
instructed to seek medical advice immediately and not enter KTPC building.
