Incident Description: Root cause:
On September 27 2021 at the forty third(43) Machine wasn’t inspected
mining base at approximately 5:00 a team of before use
miners including Mr James was operating the
Joy Lt 1051 Loader to transport the mined
bauxite to the plant during that process the
bucket on the loader broke resulting in Mr
James being shaken up and hitting his head from
him not wearing his helmet during the process
first aid was administered to him

Design Process
Problem: At class, we have a problem storing our books and

safety gear after class resulting in them being thrown on the floor

after use which hinders productivity and working space

Solution: Create an assigned locker to each student

Material needed: Wood screws tape measure saw and drill

Evaluation: The lockers will hold students’ gears books etc resulting in a cleaner and

more productive workspace

Two lamps with single pole switch controlled; switch-start fluorescent lamp with two 2-

way and an intermediate control using conduit system of wiring

Objectives:

Construct a circuit to control the different lighting loads from various positions

Distinguish between the various controls and their operation

Follow the code /regulations or standard specification with regard to the type of

installation being done


Examine the operation of the fluorescent circuit

o 20 mm conduit (metallic or non-metallic)

o 20 mm tee-box with lid/or equivalent box

o Outlet box for single-way switches

o Outlet boxes for 2-way switches

o Outlet box for intermediate switch

o Clamp-straps for conduit, Conduit ends

o Main switch-fuse or circuit breaker panel

o Ceiling lamp holders

o 1.5 mm2 single core cable (red and black)

o 0.5 mm or equivalent flexible cord

Procedure:
i. Draw a layout of the circuit

ii. Conduits were installed to accommodate fixtures

iii. Conductors were drawn to accommodate wiring shown in diagram

iv. Components of the flourscent circuit were laid out on a project board

v. Appropriate connections were made where using the aid of the circuit diagram

vi. Soldering iron were used where necessary

vii. Test for polarity earthing and continuity were done in circuit

viii. Test for correct connection to the circuit were done

Observation:
During the installation of the circuit, I recognized that the red (live) wires were connected to

switches meanwhile blue (neutral) wires and red wires to lamps and live, neutral and ground
(green) to plugs. To connect the wires to the components, the end of the wires were stripped.

three switches were used to operate the fluorescent lamps. When operating switch one (1)

lamp one (1) and two (2) were able to operate, the second switch was used to operate the

lamp. The fluorescent light was not affected by the third switch and the fourth switch was

used to turn off the lamp. Lamp one (1) and two (2) became illuminated after switch one (1)

was turned on. To help with the lighting of the bulb, a starter was placed in the beginning of

the fluorescent circuit
