DECLARATION OF AGE
I, SAMUEL UKHUEDOBAH, male, Christian, Nigerian Citizen, of 4 Uwoghien
Street, Off Goodnews Street Off Sapele Road, Benin City, Edo State, hereby
declare, make oath and state as follows:

1. That I am the deponent herein.

2. That WILLIAM UKHUEDOBAH is my son.

3. That he was born on 15th March 1994 in Benin City, Oredo Local
Government Area, Edo State, Nigeria to I and his mother MRS
ELIZABETH UKHUEDOBAH

4. That his birth was not officially registered with National Population
Commission as constitutionally required, hence this affidavit.

5. That I am deposing to this affidavit in soundness of mind and body and


free from any undue influence or coercion and that I have the requisite
capacity to so do.

6. That I do hereby declare that the above particulars are true, genuine
and authentic to the best of my knowledge.

7. That the general public should please take note as this affidavit is
needed for record and official purposes.

8. I depose to this affidavit in good faith, conscientiously believing same to


be true and in accordance with the Oaths Law of Edo State.
