ARBITRATION AGREEMENT

This agreement is made by and between [party 1] and [party 2] and acknowledges our
interests in the contract, controversy, or action described below:

Both parties do hereby agree to resolve, through binding arbitration, any dispute that we
have currently or may have in the future.

If arbitration is to take place, it shall be in accordance with the rules and procedures of
