CHANGE OF NAME (Minor)


By this deed, I the Undersigned _____(Applicant name)___ Son/Daughter/Wife of
________________ permanent resident of _____(Complete Odisha address with
pincode)___ currently residing at _____(Complete address with pincode)___ do hereby
solemnly affirm and state as follows :

1. That, I am the father/mother/guardian of the minor _____(old name)___ .


2. That, for and on behalf of my minor Son/Daughter/Ward, I wholly renounce,
relinquish and abandon the use of the former name of my minor
son/daughter/ward _____(old name)___ and in place thereof do assume from the
date hereof the name of such minor as _____(new name)___ and as that he/she
may thereafter be called, known and distinguished not by his/her former name of
_____(old name)___ but by his/her assumed name of _____(new name)___ .
3. That, for the purpose of evidencing such his/her determination I hereby declare
that he/she shall, at all times hereafter in all records, deeds and writings and in all
proceeding, dealing and transaction private as well as public and upon all occasions
whatsoever, use and sign his/her name of _____(new name)___ as his/her name
in place of and in substitution of his/her former name of _____(old name)___ .
4. That, I expressly authorise and request all persons at all times to designate and
address him/her and his/her future wife/husband and children, by such assumed
