Question1:
●Attitude -the workers attitude is based on his point of view of something and
also how they think about it. Attitude is very much important to the worker
because positive Attitude will make them more or less likely to behave safely in
the workplace.
○ currently in our company the workers are behaving in careless attitude
and following negative attitude for example The workers are carelessly walking in
the pedestrian area unawarely and got hit by the vehicle facing injury.
● Ability -it is one of the factor to be considered that it is one physical ability to do
the work safely ,the availability of correct tools and equipments and also
supporting environment .the worker should have the ability to acquire knowledge
about the safety hazarda or learning the work quickly by this he can prevent
accidents.
○ in our car parking area one worker has hearing loss problem and couldn't
understand or analyze the instructions or training about the safety control
measure so that the chances of accidents will be more .
●Training - the worker should be properly trained about the work activity by the
management and can be considered as competent worker .by doing this his
Moral confidence will be increased I doing the work and the accidents Can be
prevented maintaining safe environment without himself or others being injured in
the work place.
○ in our company there is reported that is Reckless driving is going on inside
the workplace and also reported the person is hurt and Near miss cases can be
seen.
●Experience worker -in this the company should appoint the person or a worker
with a good experience in the work by checking is previous work history so by
doing it the chances of accidents will be no production loss or accidents in the
workplace.
○ in our company the worker appointed by the company is inexperienced and
there is lack of understanding about the knowledge, work ,communication with
the contractor so accidents will be more in the work place.
●Risk perception -The competent worker will recognize and also analyze the
problem and the risk associated with the work quickly and there will be less
chance of accidents happening in the workarea.
○ in our car parking area there is a worker who is mentally unstable cannot
analyze the problem about the insufficient parking space provided due to this
there was collisions bw the vehicles causing accidents.
The investigation is carried out in different categories and they are as follow :
●To identify the immediate and root cause -The incident in this usually happens
by performing unsafe acts in unsafe conditions in the worklpace but through
proper investigation of accident done by identify immediate and root cause .
○for example in our parking area there is no proper light facility in the pedestrian
due to lack of visibility accident are happening in parking area .
Immediate cause -due to lack of visibility the person may get injured by the
vehicles .
Root cause -the company has failed to maintain or provide the resources lack of
resources.
●To records the facts of the incident -in this reason we can say when the
accidents will frequently be happening in the workplace and the person or the
worker should report and then the management should record the incident in a
Documentation factual evidence for the future. Because the workers cannot
remember every incident happening in the work place without instant reporting.
○in our company there is no such proof of accident report has not been done
by the worker and also no proper documentation is maintained by the company so
that there is more accidents daily or frequently reported in our company.
●for legal reasons -in these the accident investigation are an implicit legal duty
imposed on the employer in addition to duty to report incidents.means for legal
reasons the worker should report the accident to the management because it his
duty to be done as an employee according to the health amd safety standards
guidelines.
Immediate cause - there is accident reported in the car parking area like there is
Collison bw the vehicles and also the person got injured .
Root cause -in our company there is no such reporting done by the worker to the
manager or the concerned authorities because due to Blame culture ,rude
behaviour etc or the management is not responsive for the situation so accidents
are happening more in our company.
●staff morale - in these reason we can say non -investigation of accidents has a
detrimental effect on morale and safety culture because worker will assume that
the organisation does not value their safety.
Immediate cause -there is accident reported in the car parking area that the person
walking in the pedestrian got hit by the vehicle and got minor injury .
Root cause-the worker didn't report about the accident thinking about the
organisation's negligience ,carlessness towards safety so due to this accidents
Can happen more in the future .
●to enable risk assessment to be reviewed and updated -in this the incide
suggests a deficiency with the risk assessment which should be addressed.
Immediate cause-when the reporting of the accident is done there was no proper
investigation done on the spot by the manager and didn't review the situation and
updated it to higher authority.
Root cause - the manager negligence and carelessness about safety and he is
lacking workers coperation with the work and he is not in the site at the time of a
accident .
●for disciplinary purpose -in this reason the company instead of blaming the
worker for the accidents rather has to discipline the worker and it is their job to do so
by this their behaviour to safety culture can be increased in a positive way.
Immediate cause-due to reckless driving and under the influence of alcohol the
driver hit the person inside the parking area and got him injured.
Root cause -due to management failure by not conducted safety committee meeting
and also no proper communication with the worker and also lack of counseling to
the worker.
●Heinrichs triangle theory -it states that 88 % of all accidents are caused due to
human decision by causing unsafe acts in the wor
