Question1:

lead to the nearmiss in the


carpark.?
Individual factors including his / her competence,skills,personality, attitude
and also risk perception.individual characteristics influence behaviour in
complex ways.some characteristics such as personality is fixed ,others such as
skill and attitude may be changed or enhanced.

In our company based scenario there is a lack of safety control measures


and also the workers are lacking confidence ,skills etc
The individual human factors can be categorised as follows :
1.competence
2.attitude
3.knowledge
4.ability
5.training

8.motivation

● COMPETENCE -The worker can be said competent when he has combination


of skills,training experience,knowledge and also ability to perform the task
safely.So that near miss or accident will come to control.
○ for example in our company there is a lot of near miss and accident
are caused and the workers are not competent enough to work and also they are
lacking proper training, experience, skills which is not provided by the
management.

●Attitude -the workers attitude is based on his point of view of something and
also how they think about it. Attitude is very much important to the worker
because positive Attitude will make them more or less likely to behave safely in
the workplace.
○ currently in our company the workers are behaving in careless attitude
and following negative attitude for example The workers are carelessly walking in
the pedestrian area unawarely and got hit by the vehicle facing injury.

●Knowledge-The knowledge enables the workers to identify hazard ,prevent or


reduce and also eliminate potential accidents.so that by awareness of the
hazard. The worker will be able to judge quickly about the possible outcome of
the hazard and can prevent accident.
○for example in our company there is a welding work going on inside the
parking area and then the newly appointed inexperienced worker didn't see the
warning sign board and walking near the activity area the metal fumes produced
during welding injured the worker causing burns and cuts .

● Ability -it is one of the factor to be considered that it is one physical ability to do
the work safely ,the availability of correct tools and equipments and also
supporting environment .the worker should have the ability to acquire knowledge
about the safety hazarda or learning the work quickly by this he can prevent
accidents.
○ in our car parking area one worker has hearing loss problem and couldn't
understand or analyze the instructions or training about the safety control
measure so that the chances of accidents will be more .

●Training - the worker should be properly trained about the work activity by the
management and can be considered as competent worker .by doing this his
Moral confidence will be increased I doing the work and the accidents Can be
prevented maintaining safe environment without himself or others being injured in
the work place.
○ in our company there is reported that is Reckless driving is going on inside
the workplace and also reported the person is hurt and Near miss cases can be
seen.

●Experience worker -in this the company should appoint the person or a worker
with a good experience in the work by checking is previous work history so by
doing it the chances of accidents will be no production loss or accidents in the
workplace.
○ in our company the worker appointed by the company is inexperienced and
there is lack of understanding about the knowledge, work ,communication with
the contractor so accidents will be more in the work place.

●Skill - it is the method of understanding workers strength and limitation in the


world irrespective of the experience and skill cannot be predicted or judged.by
appointing a skilled worker there will be less time consumption during the work
and also accident or injury can be controlled.
○ in our company a worker parking the vehicle doesn't have a skill of
Reverse parking of the vehicle and hitting the compound wall. So accident is
caused .

●Motivation - in this case the company should arrange a [Safety committee


meeting] the manager and the workers should have a good communication bw
them and lot of problem will be discussed Risk assessment will be doneand by
doing this the workers feel motivated and confidence,moral of the worker will be
increased to do the work.
○ currently in our company the workers and managers is maintaining bad
relationship about the work no safety meeting are conducted. The workers are not
reporting any accidents by blaming nature lack of confidence in the workers is
seen and because accidents are happening in the parking area .

●Risk perception -The competent worker will recognize and also analyze the
problem and the risk associated with the work quickly and there will be less
chance of accidents happening in the workarea.
○ in our car parking area there is a worker who is mentally unstable cannot
analyze the problem about the insufficient parking space provided due to this
there was collisions bw the vehicles causing accidents.

●personality -it is various persons character,such as risk taking or risk


aversion ,introversion or extroversion etc.the workers attitude can be
increased and competence developed but the personality remains same .
○in our company the worker is recklessly driving under the influence of
alcohol since so many months and also reported but action was not taken like
counseling not done by the management. By this accidents will be more .

Question2: what arguments can u give to convince the committee to investigate this
car park near miss ?

Investigation in the health and safety measures is conducted because it helps to


identify the root cause of accidents caused in the work site area. And also make
sure to understand effective measures to prevent similar incident that may happen in
the future.
The investigation process is divided into two categories:
1.immediate cause -the accident caused in the worksite area .
2.underlying /root cause -the factor which is responsible for the accident caused
in the worksite area .
3.workers responsible - the worker responsible for the accident.

The investigation is carried out in different categories and they are as follow :

●To identify the immediate and root cause -The incident in this usually happens
by performing unsafe acts in unsafe conditions in the worklpace but through
proper investigation of accident done by identify immediate and root cause .
○for example in our parking area there is no proper light facility in the pedestrian
due to lack of visibility accident are happening in parking area .
Immediate cause -due to lack of visibility the person may get injured by the
vehicles .
Root cause -the company has failed to maintain or provide the resources lack of
resources.

●To identify corrective action to prevent a recurrence- this is major reason in


investigation process and also act as key motivation behind incident investigations
○ For example there is worker Found driving recklessly inside the parking area
and chance of accident is more and the person may get injured.
Immediate cause -due to reckless driving by the worker inside the parking area the
person will get injured.
Root cause- the management failed to maintain safety control measure by not
providing Defensive driving training to the driver and making way for the accident
to happen .

●To records the facts of the incident -in this reason we can say when the
accidents will frequently be happening in the workplace and the person or the
worker should report and then the management should record the incident in a
Documentation factual evidence for the future. Because the workers cannot
remember every incident happening in the work place without instant reporting.
○in our company there is no such proof of accident report has not been done
by the worker and also no proper documentation is maintained by the company so
that there is more accidents daily or frequently reported in our company.

●for legal reasons -in these the accident investigation are an implicit legal duty
imposed on the employer in addition to duty to report incidents.means for legal
reasons the worker should report the accident to the management because it his
duty to be done as an employee according to the health amd safety standards
guidelines.
Immediate cause - there is accident reported in the car parking area like there is
Collison bw the vehicles and also the person got injured .
Root cause -in our company there is no such reporting done by the worker to the
manager or the concerned authorities because due to Blame culture ,rude
behaviour etc or the management is not responsive for the situation so accidents
are happening more in our company.

●Claim management-if a claim for Compensation is lodged against the


employer ,the insurance company will investigate the accident investigation report to
help determine liability.
Immediate cause - for example in our company there is a reckless driving is
reported in the car park area and the truck hit the company wall and the company
property was damaged.
Root cause -the driver was not aware of the companies safety policy and and the
insurance company didn't issue the claim for insurance due to the failure of
maintaing the policy and also not provision training of [defensive driving training
to the worker ]

●staff morale - in these reason we can say non -investigation of accidents has a
detrimental effect on morale and safety culture because worker will assume that
the organisation does not value their safety.
Immediate cause -there is accident reported in the car parking area that the person
walking in the pedestrian got hit by the vehicle and got minor injury .
Root cause-the worker didn't report about the accident thinking about the
organisation's negligience ,carlessness towards safety so due to this accidents
Can happen more in the future .

●to enable risk assessment to be reviewed and updated -in this the incide
suggests a deficiency with the risk assessment which should be addressed.
Immediate cause-when the reporting of the accident is done there was no proper
investigation done on the spot by the manager and didn't review the situation and
updated it to higher authority.
Root cause - the manager negligence and carelessness about safety and he is
lacking workers coperation with the work and he is not in the site at the time of a
accident .

●for disciplinary purpose -in this reason the company instead of blaming the
worker for the accidents rather has to discipline the worker and it is their job to do so
by this their behaviour to safety culture can be increased in a positive way.
Immediate cause-due to reckless driving and under the influence of alcohol the
driver hit the person inside the parking area and got him injured.
Root cause -due to management failure by not conducted safety committee meeting
and also no proper communication with the worker and also lack of counseling to
the worker.

●data gathering purposes-this reason the company should maintain accident


statistics which can be used to identify trends and patterns;this relies on the
collection of good quality data.
Immediate cause -there is accident took place in parking area in welding activity
area the driver took sudden turn without knowing the situation and accident
happened .
Root cause -this accident was recorded previously there was no proper light
facility and also prohibition sign board so driver unknowingly caused the
accident.

●Heinrichs triangle theory -it states that 88 % of all accidents are caused due to
human decision by causing unsafe acts in the wor
