_______________ have been repaired to my satisfaction by ____repairer’s
name______________. For the services so rendered, the payment may be
directly released to them. Such a payment will constitute full and final discharge
of the liability of the company under my Claim No. ______________ under the
aforesaid policy.

Incase, the Insurance Company, for any reason whatsoever, fails to make
complete payment or any part thereof, I shall be liable to make payment to
repairer to the extent of the sum of money specified in the repair invoice.
