AFFIDAVIT OF GUARDIANSHIP/ABANDONMENT

I, JOEL M. POLILLO, of legal age, Filipino, single and resident of Sitio Camandag,
Mayamot, Antipolo City, Rizal, after having duly sworn to in accordance with law, hereby
depose and state, that:

1. That I am the father of JOHN CALEB POLILLO born on FEBRUARY 25, 2020
in ANTIPOLO CITY, RIZAL;

1. That my son JOHN CALEB POLILLO was born out of wedlock, his mother is
LERMA MANLAPAZ;

2. However, the mother of my son Lerma Manlapaz, abandoned our child,


the same year 2020 when our son born, my son was only eight (8) months
old that time. Since then, she never visited my son and no longer to locate
her whereabouts up to the present;

3. That I do not have any information of whereabouts of Lerma Manlapaz up


to the present and completely she abandoned our son JOHN CALEB
POLILLO;

4. That since then, my son JOHN CALEB POLILLO is under my custody, I stand
as his guardian and I shouldered all his expenses for his personal needs,
foods, medicine etc.;

5. I am executing this affidavit to attest to the truth of the foregoing facts


and for whatever legal purposes this may serve

AFFIANT FURTHER SAYETH NAUGHT.
