I voluntarily allow my son/daughter to participate in Martial Arts Festival Exhibition game.

I hereby certify that my child is physically and mentally fit to participate in this exhibition game.

I release the organizers and officials of this exhibition game for any liability whatsoever, arising from any
injury or damage which be sustained during the exhibition game and waive any right or course of action
relative thereto.

The coaching staff of my child’s team will be liable for the well-being of my child during and after the
exhibition game.
