I Jack Smith, residing at 555 14th St, San Rafael, California 94901, duly sworn, and states: 1. I am over the age of eighteen years and have personal knowledge of the following facts. 2. Susan Smith, who died on February 08, 2012 left the following items of real property and did not leave a Last Will and Testament 8 Hayes St, San Rafael, California, 94901, located in Marin County and legally described as San Rafael Bakery 3. At the time of death Susan Smiths, sole surviving heirs were as follows: Rose Smith, 12 D St, San Rafael, California 94901, a Daughter, born February 06, 1991 I declare that, to the best of my knowledge and belief, the information herein is true, correct and complete as of February 24, 2012, the date I affixed my signature to this Affidavit. ______________________________________
