Direct Deposit Form

This direct deposit form allows employees to have their paychecks deposited directly onto their prepaid debit cards. The form requires the employee to enter the amount they want deposited each pay period and select if they want the entire check amount, a percentage, or fixed dollar amount deposited. Employees must also provide their social security number, signature, and date before submitting the completed form to their payroll or benefits office.
