Description:
On 24-May-2014 at 08.30 am, While daily site inspection it is found
that the asphalt cutter was found using without fire extinguisher near
by or at the easy reach.

Immediate Cause
1. Unaware about the necessity of fire extinguisher.
2. Lack of Knowledge about the spark generation potential
3. Lack of supervision
4. Lack of HIRA communication by the supervisor

1. DCP fire extinguisher were
placed immediately at the
premises

Description:
On 16-May-2014 at around 03.00 pm, during carpentry work ,it was found that
one of the worker was using with a defective hammer which had a small crack
on the wooden handle which can result into breakage .

Immediate Cause
1. Carelessness of the worker and site supervisor
2. Hand tools were not inspected prior to work.
3. Poor supervision

1. The damaged tools were
immediately removed from the
site
2. Supervisor and the workers were
strictly instructed to do daily
inspection on the tools they are
using
