Causes of human errors
Lack of/non adherence to standard procedures:
Over confidence or Underestimation of true risk Split attention/ Lack of focus/ prone to distraction Unnecessary haste Lack of knowledge, skill or rules Lack of insight Delegation of job to unsuitable person Lack of understanding of requirement of job Ineffective supervision Poor/unclear communications Violation of Procedures Desire to take undue risks or Complacency First working day after days off Doing avoidable maintenance work in critical areas

Error in simulation by C&I engineer
Lack of co-ordination with operation Error by third party expert Operation of wrong auxiliary from OWS Wrong field connection/ isolation Poor condition of field instruments All open access of DCS in OWS
Avoidance of Human error
Whenever bypassing is inevitable it is to be done by C&I only on the request of operation and with the information and consent of the SCE. All such bypassing is to be recorded in the Interlock/ Protection bypass register. This practice will eliminate the possibility of protections remaining bypassed un intentionally. Moreover C&I must review the status of protection and interlocks on a daily basis. Interlock & protection checking for all equipments and systems including valves C&I should make all operation Engineers conversant with all the protections and interlocks Major changeovers which are likely to affect unit protections shall be done during normal working hours, as far as possible. All critical activities should be discussed and proper planning to be done considering the risk involved.
Avoidance of Human error
Have two person on critical job.
Validate the action before forcing/bypassing. Properly follow the signal for bypassing. Do not use short cut or tags. Do not work in marshalling panels in running units. Do not reset or restart network switches of running unit. Involve person who has knowledge of effect of action on process. Have clarity of communication. No deviation from proper procedure. Have valid and proper permit to work. Have proper work instruction for field instruments isolation / putting into operation. No time pressure from management
Avoidance of Human error
All desk operation work is to be done by desk engineer during work permit. Simulation of signal only by experienced engineer after due diligence with seniors Ensure that no command to be given from CER or Programmer room Before forcing ensure correct tag in point information and also correct bit no in case of packed bit forcing. Walk down of critical area once in week to assess the condition of field instruments. No modification or improvements of logic in running units
