Grade 2 Ankle

Inversion Sprain

Snapshot

Patient states that on December 12th, 2015 they were playing tennis

in preparation for tryouts when he stepped and tripped over a

tennis ball. The patient states he tripped over a ball behind him in

the middle of a point, and his right ankle suddenly twisted inward.

Patient states he noticed his ankle was moderately swelling up, pain

level 6/10, and could slightly move his ankle.


What Is It?

Inversion Ankle Sprain: occurs when the


foot is forced into inversion (turned
inward) beyond ligamentous or
muscular control.
Grade 2: Partial tearing of the ligament.
If the ankle joint is examined and moved
in certain ways, abnormal looseness
(laxity) of the ankle joint occurs.
Common injury in high intensity athletics
such as basketball, football, and soccer,
and tennis.
Signs and Symptoms

Include: Pain, swelling, bruising,


tenderness, discoloration
Sometimes one can hear or feel a
snap, pop, or tear
A lot of pain at first, hard to walk and
put weight on injured foot
Special Tests

Anterior Drawer: 10-15 degree


plantar flexion of the foot, tests for
instability of the ankle. Test for the
Anterior Talofibular Ligament (ATFL)
Talar Tilt: Detects excessive ankle
inversion. Gentle inversion. Test for
the Calcaneofibular Ligament (CFL)
Treatment

R.I.C.E.: Rest ankle by not walking on it,


ice to reduce swelling, ace
wraps/bandages to support ankle,
elevate ankle above heart
3 Phases:
1. Resting and protecting (one week)
2. Restoring ROM, strength (1-2 weeks)
3. Return to activities without turning or
twisting (2 weeks-months)
Prevention

Maintain good strength, muscle


balance, flexibility
Always do warm-ups, stretches
Wear appropriate shoes
Do not over train, know what your
body can handle
