FHP (Gordon's Approach) Format

Gordon's functional health patterns (FHP) is an assessment approach developed by Marjorie Gordon to guide comprehensive nursing data collection. It enables nurses to evaluate 11 aspects of a patient's health and functioning, including health perception and management, nutrition, elimination, activity, sleep, cognitive functioning, self-perception, roles and relationships, coping abilities, sexuality, and values. The FHP approach involves collecting data on a patient's history, current symptoms, medical diagnoses, vital signs, and the 11 patterns to understand their overall health status.

 Proposed to guide for establishing a comprehensive nursing
data base
