Fall From Height

A colleague fell through a roof while working at height after a roof tile broke under their weight, though their safety line prevented a full fall. An investigation found that access to the roof was unsafe, there was a poor safety culture as this was the third similar incident, and required safety assessments and checks were not completed. New processes will be put in place requiring permits for high risk work, ensuring safety nets are used, and that only trained employees perform such tasks after a proper risk assessment.

Fall From Height

WHAT HAPPENED?

A colleague fell through a roof while working at height. They


lost balance whilst standing on a walkway and stepped out
onto roof tiles which broke under their weight. Our colleague
was wearing a safety line which stopped the fall and our
colleague was forced to perform a self-rescue.

WHY?
• Access provided to the roof was unsafe, but our team Hole in roof where tile broke
proceeded with the work anyway
• Poor safety culture (this was third similar incident)
• The Risk Assessment, Safe Systems of Work & 3 Checks
For Safety all failed.

WHAT WE NEED TO DO?


• Remind all managers and teams that work at height is a very high risk activity
• High risk activities must only be performed by appropriately trained and experienced people
following correct process including a risk assessment.
• A ‘Permit-to-work’ must be issued before work starts.
• All Sodexo Safety Nets must be ‘in place and working’ especially for all high risk activities;
before any work is started
