How unsafe act can causes


serious accident ????
An Ironworker crew was modifying a temporary structural steel bridge
using an Oxygen/Acetylene torch setup on a cart, a portable welder,
and a grinder. Apparently the Acetylene hose developed a leak and
ignited during the grinding operation
The torch setup was not equipped with a “flashback” arrestor
check valve and the fire traveled through the hose into the
compressed cylinder.

All Oxygen/Acetylene setups must


have a “Flashback” arrestor check
valve at the regulators, NOT the
torch head, to prevent this type
of occurrence. This little device would
have prevented this fire.

What kind of accident is about to happen?

A wire is attached to the hook of a drag shovel with a crane attachment, and
is being used to move a steel arch support at a tunnel construction site.

Look what happened!
The shovel operator stopped the shovel suddenly since a worker walked
under the overhead load. As a result, the wire rope came off of the hook and the support
beam fell on the worker.

TIPS FOR PREVENTING SIMILAR ACCIDENTS

• Never let a worker walk under


overhead loads.

• Be sure to inspect the safety catch for the


hook before starting work.

• Assign someone to serve as a signaler


and perform work in accordance with that
persons instruction

• Know proper methods of hanging support


beams and the order in which beams are
to be installed

At a construction site of burying city water pipes, a hydraulic excavator is traveling with a pipe
weighing 53 kg that is hung with a wire rope fixed at the center. A worker is holding the pipe with
his hand to prevent it from swaying.

Now what accident do you foresee occurring in this frame?

This is the accident resulted!
The operator suddenly changed the course to avoid a big stone ahead. Then the hung
pipe made a big sway, pulling the worker down. He was run over by the excavator and
killed.
Tips for forestalling similar accidents

• Strictly follow the rules restricting the use of


hydraulic excavators in unintended
applications (see the Article 164 of Rules of
Industrial Safety and Health Law).

• Take every precaution for safety at the


worksite beforehand. In this instance
remove, remove big stones lying on the
planned traveling course

• Train operators thoroughly not to jerk


hydraulic excavators as well as their work
equipment.
