1. Children born to a Filipino mother without foreign documents such as
medical/hospital record of birth certificate from the country where the
event occurred:
a. Affidavit of the mother stating the circumstances and facts of birth
of her child; and
b. Affidavit of two disinterested persons who can support the facts
given by the mother

2. Children born to a Filipino mother without foreign documents such as

where the event occurred, and the mother is deceased but with known
facts of birth and the custody of the child is left with the father, legal
guardian or any person whom the child was entrusted:
a. Affidavit of the father, legal guardian or any person whom the child
was entrusted to; having personal knowledge of the facts of birth of
the child and
b. A copy of the Certificate of Death of the mother.
c. Affidavit of two disinterested persons who can support the facts given
by the informant

4. Children born to Filipino mothers without foreign documents such as
medical/hospital record of birth or local birth certificate from the country
where the event occurred, and the whereabouts of the mother is
unknown or the mother is missing and the custody of the child is left with
any person who are not aware of the facts birth of the child:

The Philippine Foreign Service Post may conduct further investigation as


to the facts of birth of the child. In case of unsuccessful compliance of the
above mentioned requirements, the child shall be registered pursuant to
the existing laws of the country where the child was born.

Supplemental Memorandum Circular to MC 2017-08 (Full Implementation
on the Use of 2007 Revised Civil Registry Forms)

The CRG already made initial and minor revisions on the 2007 Revised Civil
Registry Forms for birth, death and marriage certificates covering the following
items:

1. Item # 25 in the COLB – “Registered by the Civil Registrar” to “REGISTERED


AT THE OFFICE OF THE CIVIL REGISTRAR”
2. Item # 22 in the COM – “Registered by the Civil Registrar” to “REGISTERED
AT THE OFFICE OF THE CIVIL REGISTRAR”.
3. Item # 29 for the COD – “Registered by the Civil Registrar” to
“REGISTERED AT THE OFFICE OF THE CIVIL REGISTRAR”

Full Implementation on the Use of 2007 Revised Civil Registry Forms

Therefore, the use of old civil registry forms is no longer acceptable.
There are new items and revisions included in the 2007 Revised Civil Registry
Forms that are not present in the old forms.

Issuance of Original and Certified True Copy of Certificate of Live Birth,
Certificate of Marriage and Certificate of Death

Original and certified true copy of Certificate of Live Birth, Certificate of Marriage
and Certificate of Death, can only be issued to:
1. The owner himself or through a duly authorized representative;
2. His/her spouse, parent, direct descendant, guardian or institution
legally in-charge of him/her, if minor;
3. The court or proper public official whenever absolutely necessary in
administrative, judicial or other official proceedings to determine the
identity of a person;
4. In case of the person’s death, the nearest of kin.

Additional Guidelines in Preparing and Issuing Supplemental Report

1. If birth the Philippine Statistics Authority (PSA) and Local Civil Registry Office
(LCRO) file copies contain no entry/entries whatsoever on any item in the COLB,
COD, COM and COFD, except for those mentioned under Rule 11(1), A.O. No. 1
series of 1993, a Supplemental Report is applicable to supply the missing
entry/entries in both the PSA and LCRO copies.

2. In case there is a difference in the entries between the PSA and LCRO copies,
illustrated as follows:
a. Where there is an inadvertently omitted entry in the PSA-copy but the
corresponding information is available in the LCRO-copy; and
b. Where there is an inadvertently omitted entry in the LCRO-copy but the
corresponding information is available in the PSA-copy

Supplemental Report shall be applied on the affected document

3. A Supplemental Report is also applicable to supply the missing the


entries in the Geographical Portion of the documents.
Error/errors in the Geographical Portion of the documents shall be
corrected using the procedures outlined under Memorandum
Circular No. 2010-4.

4. A Supplemental Report is not applicable to supply the registry


number on the PSA Copy which contains no registry number, the
Local Civil Registrar shall endorse a certified copy and/or transcription
of the document with the correct registry number.

5. If both “Male” and “Female” are marked “X” on the entry on Sex, a
Supplemental Report shall be filed to correct the entry.

6. For blank entry on the year of birth, a Supplemental Report or


petition under RA 10172 shall not apply. Missing year of birth shall be
supplied by filing a petition in court.

Implementation of the Feedback Templates

Feedback Form is used to applications denied or disapproved for processing


due to lack of requirements, or non-compliance to the business rules, or
inconsistencies in the entries.

PAID TRANSACTIONS/APPLICATIONS – the submitted documents together


with the feedback form are returned to the requesting clients.

MONTHLY SUBMISSION –the documents submitted in the monthly reports


or endorsements from the LCRO, the Feedback Forms together with the
supporting documents are filed at the PSA document archives for retrieval in
case the LCRO or the client complied with the requirements.

1. Conduct of measurement must be done every month.
2. Submission of reports (Room Temperature Measurement Form, RSC
Measurement Form and Toilet/Cleanliness Measurement Form) must
be submitted a week after the reference month
3. Softcopy of the signed report must be emailed in advance to the CRS-
ITP2 Project Director, Chairpersons of Business Process Sub-
committee
4. Accomplish and submit signed copies of the Measurement Forms in
three (3) copies

ROOM TEMPERATURE
1. Assessment will be done on 10 random observations
2. The 10 random schedules (date and time) are to be identified by the
Outlet Manager (OM). This schedule will be kept secret from the
outlet supervisor (OS) and the designated Unisys Team Lead (TL) until
the observation day itself
3. OM will advise the OS and designated Unisys Team Lead (TL) of a
temperature measurement schedule on the observation day itself.
The designated TL is to be informed immediately by the OS upon
getting the instruction from OM.

ROOM TEMPERATURE
4. On the advised Temperature measurement date and time, the OS (or
his/her representative) in the presence of the TL (or his/her
representative) shall measure the temperature at 3 random points on
the outlet’s area and main customer area using a digital
thermometer. The average of these 3 reading will be the outlet’s
temperature for the scheduled observation day.

Toilet/Restroom Condition

1. Within the first week of the month, five (5) days during the month is
to be picked by the OM when the toilet/restroom cleanliness survey
will be conducted. The OS is to be advised of the schedule.
2. The designated Unisys TL is to be informed immediately by the OS
upon getting the advice from OM.
3. Measurement for toilet/restroom is to be conducted two (2) in the
morning, one (1) during lunch and two (2) in the afternoon.

4. On the designated survey date, five (5) clients are to be randomly


selected by the OS (or his/her representative) in the presence of the
Unisys designated TL.
5. A ¼ sheet survey form is to be distributed by the OS wherein the
client will rate the cleanliness of the toilet/restroom. The form
contains the name and the signature of the respondent

Marriages Solemnized under Civil Code or Family Code and Dissolved
through Divorce after Conversion to Islam by One of the Spouses

1. The provision of this title shall apply to marriage and divorce wherein
both parties are Muslim or wherein only the male party is a Muslim
and marriage was solemnized in accordance with Muslim law of this
Code in any part of the Philippines.
2. In case of marriage between a Muslim and non-Muslim solemnized
not in accordance with Muslim law or this Code, the Civil Code of the
Philippines shall apply.

Note: All civil marriages of non-Muslim spouses who either one or both of
them were converted to Islamic religion cannot invoke PD 603 as these
where not covered by said law.

ANNOTATION

“Pursuant to the Recognition of Divorce Decree dated (date of decision)


rendered by (Name of the Judge) of the (name of court) the marriage
between (name of spouses) is hereby declared as dissolved/divorced.”

In case of marriage between a Muslim and a non-Muslim, solemnized not


in accordance with Muslim law of this Code, the Civil Code of the
Philippines shall apply. In this case, no annotation shall be done on the
Certificate of Marriage.

CASE NO. I
The additional name “Jr” was not entered (omitted) in the COLB to
distinguish the father from the child. The petitioner may apply for
supplemental report under Rule 11 or AO 1 Series of 1993, provided
that the entry in the father’s first and last name bear similar first and
last name of the child. This is pursuant to Article 375 of the Civil Code.

Likewise, the additional name II, III, IV if omitted can be supplied by


applying the procedure under Rule 11 (Supplemental Report), AO No. 1
Series of 1993. Prior to the effectivity of AO 1, series of 1993, we
considered the first name “Baby Boy” and “Baby Girl” as if names were
omitted, hence these are cases falling under the procedure of
supplemental report.

The additional name “Jr” was erroneously entered in the COLB as


part of the child’s firt name but the father’s first name is different
from the child. “Jr” can be considered erroneously entered.

The proper petition to be filed is a petition for correction (CCE) of


child’s first name to delete “jr.” as provided under RA 9048

The Roman numeral “II” (instead of JR) is inadvertently placed as


part of the child’s first name in the COLB.

Under this case, the additional name to be entered should be


“Jr” not II. A petition for correction of clerical error is proper
under this case.

The additional name “JRA” or the use of Roman numeral “II” was
inadvertently placed in the child’s name who is a female.

There is no legal basis for a daughter or other female


descendants to use the additional name “JRA” or placing any
Roman numeral with their first names. Art. 375 of the Civil Code
applies only to the “son, grandson” or “other direct
descendants”.

If the name of the child is written in full as “JUNIOR” and it is to


be changed to short ‘JR”, and vice versa, the proper petition to
be filed should be a petition for Change of First Name (CFN).

CASE NO. VII

If the additional name “Sr./Jr.” is to be entered in the father’s


name either in the COLB and Certificate of Marriage (COM) ,t he
same must be subjected to the filing of a petition for correction
of clerical error (CCE) under RA 9048 and not through the
procedures of supplemental report under AO series of 1993
provided the “Sr./Jr.” appear in the father’s COLB.
