APPROVING THE STANDARD SPECIFICATIONS FOR
POLICE BLOTTER
INTRODUCTION

The specification of Police Blotter was set by the National Police


Commission in order and conformity with the requirements of Republic Act
6975, as Amended by Republic Act 8551, its Implementing Rules and
Regulations (IRR), all related logistics policies and issuances of the
Commission, to meet the requirements of the PNP.

BLOTTER GUIDE
1. References:

2. Purpose:

To prescribe a uniform police blotter for the Philippine National Police


and to define the rules and procedure of making entries therein.
3. Definition

A police blotter is a record of daily events occurring within the


territories/ jurisdiction of a given police unit or office. It contains material
details concerning the event for legal and statistical purposes. This police
blotter is therefore an informational record book that is utilized for
evidentiary or referral purposes.
4. Form and Size:

The police blotter shall be a record book bound with hard covers and
shall have the following dimensions:

Length – 40cm
Width - 30 cm

5. Contents of Entry:

The entry in the police blotter should answer the following cardinal
elements of a police record, to wit:
(1) Who (5) When
(2) What (6) How
(3) Why (7) Disposition of Case
(4) Where
Contents of Entry:

In answering the five (5) Ws and one (1) H and the Case Disposition, all
such material details about the event, including the names of the suspects,
victim and witnesses if any; the nature of the action or offense; the possible
motive; the place; the date and time of occurrence; significant circumstances
that aggravate or mitigate the event of the crime; should be entered along with
the identity of the officer to whom the case is assigned (Officer-On-Case); and,
the status/ disposition of the case. The officer shall write his name; put the date
beside it and sign above.
Contents of Entry:

The Duty Desk PNCO of the office who receives the report of an
incident/ transaction shall record it in the police blotter following the
cardinal elements of a police record.
The following incidents or transactions, among others, should be entered in
the police blotter:

(1) Violations of law and ordinances reported and/ or discovered;


(2) Call for police assistance, dispatches and action/s taken relative to the assistance
sought by the caller;
(3) Fire alarms, reports and information received by the stations;
(4) Movement of prisoners with corresponding notations on the authority for such
movement;
(5) Cases of missing and/or found persons, animals and property;
The following incidents or transactions, among others, should be entered in
the police blotter:

(6) Traffic accidents and violations which require police action;


(7) All personal injuries, bodies found, and suicides;
(8) Damage to property;
(9) Cases in which a police member/s is/ are involved; and
(10) Warrant of Arrest received and served including those unserved and returned to
issuing office
6. Guidelines in Making Entries:

a. All entries in the police blotter shall be handwritten in a clear, concise


and simple manner but answering as far as practicable the 5 Ws and 1 H.
Clarity should not be sacrificed for brevity.

b. Only facts, not opinions, are entered in the blotter.

c. No erasures shall be made on the entries. Corrections are made by


drawing one horizontal line overs such word or phrase and the actual entry
initiated by the police officer making the correction.

d. A ball pen with blue or black ink should be used in making the entries.

e. Misrepresentation in the blotter or any attempt to suppress any


information therein is punishable criminally and administratively.

f. The entries must be legibly written in print and consecutively


numbered.

g. Every page of the blotter shall be consecutively numbered


chronologically and fully filled-up. No line or empty space shall be left
blank between any two entries. Empty space in the last line of entry shall be
marked with “X” until the end of the margin of that line.

h. The Duty Desk PNCO shall print his complete name, rank, designation
below the last line of entry and to be duly signed.
Guidelines in Making Entries:

i. Any development of a case to be reflected in the blotter should be a new entry


at the time and day it was reported. A reference to the previous entry number of the
case shall, however, be made.

j. In every shift, the Duty Desk PNCO under the supervision of the Duty
Officer complaint Desk Officer, shall make the actual entries in the blotter at the
end of his tour of duty, both the Duty Desk PNCO and Duty Complaint Desk
Officer shall sign the blotter.
7. Units Required to Maintain the Blotter:

a. Every police station and/ or precinct shall maintain a police blotter.

b. All PNP operating units or City/ Provincial/ District Police Offices. In


addition to station/precinct shall likewise maintain separate blotters.
8. Supply and Accountability:

a. PNP Headquarters shall supply police blotters to each PNP command/


unit who shall carry the same in their property books as an accountable item.

b. The PNP unit Commander shall be responsible for the proper


maintenance, use safekeeping and accounting of police blotters.

c. Pending the procurement and issuance of the uniform police blotter,


police units shall continue to use the present blotter, provided the forms of
entries and the rules for the maintenance thereof as stated above shall be
followed.
