ANECDOTAL RECORDS

Presented by:
Ms. Monika Makwana
M.Sc. (N) 1st Year
GENERAL OBJECTIVE

At the end of this topic, students will be able to gain in depth knowledge regarding
Anecdotal record.
SPECIFIC OBJECTIVES

At the end of the topic students will be able to:

• Define anecdotal records.
• Determine the characteristics of anecdotal record.
• Appraise the purposes of anecdotal record.
• To know about the advantages and disadvantages of anecdotal record.
• Recognize the uses of anecdotal record.
• Explore the guidelines for making the anecdotal record.
• Recall the basic elements for formulating the anecdotal record.
• To formulate a anecdotal record independently.
INTRODUCTION

• Anecdotal or progress notes are objective written descriptions of observed student

performance or behavior.

• They are the factual description of the meaningful incidents and events that the teacher
has observed while dealing with the students.

• It is also used for describing a nurse's experience with a person or group especially in
the clinical setting. Each incident should be written down shortly after it happens.

DEFINITION
• Anecdotal records can be defined as a brief description of an observed behavior that
appears significant for evaluation purposes. It is a factual record an observation of a
single specific, significant incident of the behavior of a student

• Anecdotal records are objective descriptions of behavior recorded on plain paper or

a form. The notation should include who was observed, by whom, when and where.
The notation comprises a description of the setting or background and the incident
and interpretation and recommendations may be included. Value laden words such
as good and bad should be avoided.
DEFINITION

According to Randall, Anecdotal record is a record of some significant item of

conduct, a record of an episode in the life of students, a word picture of the student in
action, a word snapshot at the moment of the incident, any narration of events in
which may be significant about his personality.
Conti…..

• The format for these can vary from loosely structured “plus/minus" observation notes
to structured lists of observations in relation to specified clinical objectives. They may
be recorded on separate cards or as running accounts, one for each student, on separate
pages of a notebook

• A good anecdotal record keeps the objective description of an incident separate from
any interpretation of the behavior's meaning. These written notes initially serve as part
of formative evaluation. As student performance records are documented over time, a
pattern is established.
MEANING

• Informal devices used by the teacher to record behaviour of students as observed

by him/her from time to time.

• It provides lasting record of behaviour which may be useful later in contributing to

a judgment about a student.
CHARACTERISTICS OF ANECDOTAL RECORDS

• It is a factual recording only of the actual event, incident or observation, uncolored by

the feelings, interpretations or biases of the observer.

• It is a record of only one incident.

• It is a record of an incident which is considered important and significant in the growth
and development of the student.
Conti….

• It should include:

 a description of a particular occasion.
 a delineation of the behavior noted, indicating who, what, why, when, where and
how.
 the evaluator's opinion or estimate of the incident or behavior.
Conti….

The incident recorded should be that is considered to be significant to the students

growth and development for example:
 Simple reports of behaviour.
 Result of direct observation.
 Accurate and specific.
 Gives context of child’s behaviour.
 Records typical or unusual behaviours.
PURPOSE

• To furnish the multiplicity of evidence needed for good cumulative record.

• To substitute for vague generalizations about students specific exact description of

behaviour.

• To stimulate teachers to look for information i.e., pertinent in helping each student
realize good self-adjustment.

• To understand individual’s basic personality pattern and his reactions in different

situations.
Conti….

• The teachers is able to understand his/her student in a realistic manner.

• It provides an opportunity for healthy student-teacher relationship.

• It can be maintained in the areas of behaviour that cannot be evaluated by other

systematic method.

• Helps the students to improve their behaviour, as it is a direct feedback of an entire

observed incident, the student can analyse his behaviour better.

• Can be used by students for self-appraisal and peer assessment.

ADVANTAGES OF ANECDOTAL RECORDS

•Anecdotal records can be maintained in those areas of behavior that cannot be
evaluated by other more rigorous, systematic methods.

• It is useful in supplementing and validating observations made by other means.

• It can be used by students for self-appraisal and peer assessment.
Conti….

• It is easy to develop and highly economical . It does not require any mechanical
resources or equipments.

• It can be used for providing formative feedback. • It provides insight into the
behavior of the students.

• Frequent and brief observations make it possible to discover trends of behavior
over a period of time.
DISADVANTAGES OF ANECDOTAL RECORDS

• It is a highly subjective measure to assess the behavior of the students.

• There is a total lack of standardization.

• It is quite difficult to score the recordings.

• It is time consuming and has very limited applications.

• Interpretation of the recordings is very difficult and differs from one person to
another person, thereby making evaluation difficult.
Effective use of anecdotal records could be made by:

• Determine in advance the behavior to be assessed, and limit observations to those

categories or qualities.

• Record enough of the situations to decrease subjectivity.

• Record the incident as soon as possible after its occurrence.

• Minimum time gap to record is necessary to avoid too much dependence on memory.
This will increase its objectivity, validity and reliability.
Conti….

• Limit each anecdote to a brief description of a single specific incident

• Record both positive and negative incidents and consider both while making
inferences.

• Relate anecdotal records directly to the clinical objectives.
USES OF ANECDOTAL RECORD

• Record unusual events, such as accidents.

• Record children’s behaviour, skills and interests for planning purposes.

• Record how an individual is progressing in a specific area of development.

• It provides a means of communication between the members of the health care team and
facilities coordinated planning and continuity of care. It acts as a medium for data
exchange between the health care team.

• Clear, complete, accurate and factual documentation provides a reliable, permanent

record of patient care.
GUIDELINES FOR MAKING ANECDOTAL RECORD

• Keep a notebook handy to make brief notes to remind you of incidents you wish to
include in the record. Also include the name, time and setting in your notes.

• Write a record as soon as possible after the event. The longer you leave it you write
your anecdotal record, the more subjective and vague the observation will become.

• In your anecdotal record identify the time, child, date and setting.

• Describe the actions and what was said.

• Include the responses of other people if they relate to the action.

Conti….

• Describe the event in the sequence that it occurred.

• Record should be complete.

• They should be compiled and filled.

• They should be emphasized as an educational resource.

• The teacher should have practice and training in making observations and writing
records.
Basic Elements of Anecdotal Records

• Students name, class and section

• Date of observation
• Name of observer
• Setting or background of the information (unit/ ward/ departments)
• Incident
• Signature of the observer
• Interpretation of behavior or Brief report of what happened
• Recommendations concerning behavior.
SUMMARY

Anecdotal Record is a type of tool in which important incidents are recorded based
on factual data. In anecdotal record description of incident is recorded immediately
after the incident by the counsellor. It gives information regarding student’s attitude,
habits, interest, adjustment, etc.
