Introduction

 The home visit is the ideal place to provide proper care to


the needy person. Home visiting is very essential in
community health Nursing. The benefits of home care based
on familiarity and being surrounded by family.
 The client can be empowered as choices involving food,
treatment, medication schedules interacting with family,
friends, leading to a feeling of security and well being.
History of Home Health Care

 Home Health Care started in India during British period


(1757-1947), i.e. in the 18th century. It was begun in
India by the foreign missionaries with efforts to train
dais and midwives to conduct the deliveries in home
settings.
 1918, lady health visitor course started by Government
of India at Delhi.
Concept of Home Visit
 In India as a community health nurse she/he should have weekly
fixed program in different areas &villages. In case of emergency
tentative visits can be planned and arranged.
 Her visits should be included the following services to pregnant
mothers, intranatal mothers, postnatal mothers, new born care,
infants care, toddler care, preschool children, Anganwadi,
communicable disease and non communicable disease .
Principles of Home visiting

1. Home visiting should be made according to the needs of the


people .
2. It should be part of a planned visiting programmes.
3. Collect background information regarding the family in
particular and community in general- that is information
recording the family size , occupation, income, religion,
resources, customs and cultures.

6. In health teaching; be sure of what you discuss, i.e. scientific
soundness.
7. The approach to the family should be kind and courteous with
a view to gain their confidence.

1.Plan the home visits

 As a community health nurse , she should have the total


knowledge of the problems and care needed by each family
that means know the physical setup, cultural background
practices, beliefs, available resources, level of understanding,
socio economic condition of each family and their relation,
prioritize the care.
2.Arrange/equip the community bag
 Depends upon the care through many procedures, for
example, hygienic care, taking height and weight of under
five children, checking vital signs of the client , assessment
of pregnant women, testing the blood, urine and sputum,
conducting deliveries, immunization of pregnant women
etc.,
3.Approach the right family
 Knock the door, introduce yourself inform the purpose of visit(if it
is first time), establish rapport maintain the level of understanding.
 Be calm , polite and friendly
 Being a good observer, assess the family & background, problems,
needs etc…
 Listen their problems with eagerness and should not force them.
 Recording any information.
 Understand their customs-good and bad, make actions according
to the needs of the family.
4.Interview

 Interview can be defined as a conversation with a purpose, the


goal and objectives of interview must be informed as early as
possible.
 It is an exchange of ideas/views between two people. The
interview tool must be prepared well in advance.
 The community health nurse should be sensitive to others
feelings and with respect, should collect the real facts through
interview technique.
Principles of interview Technique

1. A community health nurse should establish good rapport


2. She /He should take informed consent
3. Provide privacy when conducting interview.
4. Maintain alertness and shows positive attitude towards client.
5. Be a good listener.
6. Should go down in the level of understanding.
7. Should know and understand the total
background of the family.

5.Interventions
 Assess the client/family thoroughly.
 Make nursing diagnosis in a constructive manner
 Plan nursing care, procedure according to the needs of the family
 Provide nursing care with comfort.
 Prepared/ make sure with the resources which is available in the
community bag /community settings.
 Assure the client /family appropriately
 Evaluate the care with the objectives.
 Record the provided care properly.
6.Closing of the visit
 Usually, a community health nurse starts her visits by 9 am and ends by
2 pm. The regular visit include important houses, specific need base
houses, example antenatal term cases, intranatal cases, neonatal,
postnatal. cases, family planning cases, old age care, Anganwadies,
schools and leaders houses during planning the programmes.
 Community people will have an understanding and coordination with
the nurses fixed days visits and objective of visits.
 If there is any programmes ,she will inform the advanced date , place ,
venue ,etc.
7.Recording the visits
 After the visits to particular area/village. Community heath
nurse should record the events example eligible couple
register , birth register, immunization register, malarial
blood smear register , death register, antenatal register,
postnatal register, family planning methods register, etc.

 Then she should write the daily diary of the visit and the
achievements, advantages disadvantages, etc.
Advantages of Home Visit

1. As a health team members, she can meet the people in their


own settings.
2. One can assess the real problems easily.
3. She/he can plan the problem solving approach along with the
community people.
4. It will create approach relationship between a nurse and family
members

5. Home visit provides an excellent opportunity to
implement the nursing process.
6. It is convenient for the clients to discuss their problem
and solving the problem easily.
7. Health teaching will be more clear in the home settings
and prevent more problems.

8. It will reduce the acute shortage of health providers and
decreases the nurse patient ratio.
9. The family people can recognize the responsibility and
practices of a heath team member.
10.As a health team member it provides an opportunity to
look for potential problems or upcoming needs.
