allowed to enter the laboratory room
with the presence of the Laboratory
Instructor.

2. All laboratory teachers and


students shall accomplish the
Health Declaration Form or any
digital iteration upon entering the
school premises before
entering/using the laboratory room.
The Health and Safety Officer will
be screening the accomplished form.

3. All laboratory teachers and
students will observe physical
distancing (at least 1.5m between
you and others), hand hygiene,
and respiratory etiquette. There
are safety barriers/partitions in
the middle of workstations to
avoid human interaction.

4. All laboratory teachers and


students should disinfect their
shoes using sanitizing foot baths
provided at the entrances.

5. Observe distinct signage for
entry and exit doors, signage of
health and safety reminders, and
a station for hand sanitizer or
alcohol by the door.

7. Laboratory Instructors and students
are required to wear their complete
Personal Protective Equipment (PPE)
comprises laboratory gowns, masks,
gloves, head caps, and goggles, during
the experiment. If they will go out of
the room, the PPE’s should be taken
off except for the mask.

8. All bags and utility boxes shall be


placed in the designated area. 

9. Eating, loitering, and playing inside


the laboratory are strictly prohibited.

10. All students will be liable for any damage to any laboratory
equipment/ apparatus due to their negligence. 

11. Students should keep away from the chemicals, laboratory


equipment/apparatuses that are not to be used during the
demonstration. 

12. Taking out of laboratory equipment/apparatuses is STRICTLY


PROHIBITED without prior permission from the Laboratory
Services Coordinator or the Laboratory Staff.

13. Observe proper waste disposal
in accordance to their respective
categories, i.e. acid, base, sharps,
infectious. (For infectious sharps
and glass, domestic wastes, and
hazardous chemicals – specific
containers are provided).

14. Students are advised to provide


hygienic materials for personal use
such as alcohol/sanitizers, extra
face masks, tissues, etc.

16. Students should disinfect the working area before
and after use. Do not left/store your belongings in the
laboratory.

17. Use of cell phones and any gadgets is prohibited


during laboratory experiments/activities. Taking
pictures inside the room is prohibited unless it is
instructed by the teacher for post laboratory
discussion purposes. Posting photos to any social
media are not allowed. Ex: selfie, groufie, picture or
video of procedures and chemicals.

1. Students must fill-out a request form for the materials not available in
the Permanent Request Materials that will be used in their
experiments.

B. Laboratory Breakages/Losses
2. Students charged for any breakages/cracks or losses of
equipment/apparatuses should fill-up the breakages/losses form and
will be submitted immediately to the laboratory staff. It will be filed
as part of the end report for the semester and basis for the signing of
student’s clearance.

2. Students can either replace or pay the said apparatus/es to the


cashier. The charge will be based on the brand and the current price
of it.

2. Laboratory instructors will be notified through text or email as soon as the


request is approved and prepared.

3. Laboratory Instructors are allowed to use any equipment/apparatus for


demonstration with prior approval of the Laboratory Services Coordinator
and the College Dean.

Group Work (Act 1&2)
•Students will be given an activity and they need to
answer it as a group. But they must still submit an
individual output.
•The score for their individual output will be base on
the correctness and completeness of their activity.
•Activity 1 and 2
•In this activities every subgroup has a unique activity.
(Same topic but different questions per group)
•As a group the student need to prepare a PowerPoint
presentation and need to present their output to the class.

•Student must submit their individual output of the activity


according to the set schedule by the instructor.

• There will be a deduction if he/she fail to submit the


output at the given time (unless he/she will present a
valid reason to instructor).(half of the score if submitted
late)
Group Work Reporting
• Student needs to present their group work
output to the class. (Group work presentation)

• All students must listen to every reporter and


discussion. Expect quiz at the end or next
meeting after group work reporting.
• During Group work presentation, the instructor
will use rubrics 1 – Rubrics in oral presentation in
laboratory (30pts)
• Student must be conscious with their allotted
time.
• If student is absent during reporting, he/she will
be automatically 0 unless proven that he/she
imparted some efforts in the activity. If proven, it
depends on the decision of the instructor.
• Every student is encourage to participate
through asking or answering questions. It will be
added to your score in reporting (individual).
Pre-Lab Discussion
• The instructor will discuss the activity and give
some instruction especially the deadline of the
activity.
Actual Experiment (Subgroup Act)
• Every students must have his/her own hard copy of Laboratory Activity.
• Students must have a complete PPE during Laboratory
• Every student must also have his/her own hygiene kit.

- Mask – use different mask every lab time


- Laboratory Gown –wear it every lab time No PPE Apparatus,
- Head Cap No experiment

- Lab Shoes Can go inside the lab
- But not allowed to conduct

• Each group must prepare the following
- Big Notebook (optional to instructor)
- Tissues (5pcs)
- Masking Tape
- Marker (Permanent & White Board)
- Alcohol
- Dishwashing Soap
- Sponge
- Rags (3pcs)
- Syringes (10pcs- 10cc, 10pcs-5cc, 10pcs-3cc)
• There will be checking of attendance, PPE and
material during actual laboratory. (Secretary of the
subgroup)
• Student must expect that there are activities in
which they will provide the materials such as fruits,
milk, eggs and others.
• Students performance in laboratory will be scored
base on Rubrics 3- Actual Laboratory Rubrics
• Every members Must have a Hard Copy of
Laboratory Activity
• During the actual experiment, the secretary of the subgroup must
check all the apparatuses they use. (before and after the experiment).
Use the Contract Forms provided to the group in every experiment.

• Students are allowed to take pictures and videos of their experiment.


BUT NO SELFIE OR GROUP PICTURE. The use of phone camera is
only for documentation purpose.

• Before Leaving the Laboratory, make sure that the area is clean and
the apparatuses used are dry. The instructor will check first the area,
apparatuses, big notebookand contract form before they are allowed
to go out of the laboratory
• There will be a deduction if he/she fail to submit
the output at the given time (unless he/she will
present a valid reason to instructor).
(-3 If submit within 3 days late from the deadline. -
10 if submit late beyond 3 days)
• There will be a deduction if he/she fail to submit the
output at the given time (unless he/she will present a valid
reason to instructor).(half of the score if submitted late)

• The score for their individual output will be base on the

POST LAB REPORTING (Act 3-16)
• The subgroup who is in charge of the activity will present the post Lab report.

• Student must be conscious with their allotted time.

WORK SHEET/ ACTIVITY


- You can print the worksheet but you must hand-written your
answer.
- Student that don’t have printer can copy and answer the Pre Lab
and Post Lab output in a short bond paper. When you are done
answering  take a picture or scan your activity (make sure it is
still legible) and submit it in schoology.
- Do not forget to WRITE your NAME and DATE in EVERY
PAPER you use and make sure that your name and date are
visible in the picture or scanned document that you are going
submit. And requesting also to please write LEGIBLY. (I will not
accept your output if you fail this part of instructions)
INSTRUCTION TO SUBMIT GROUP PRESENTATION
REPORT
• Submit your power point presentation on or before deadlines. (There
will be a group deduction of -3 if submitted late)
• Submit your group report in schoology.
• The content of your report must contain the ff:

• At the end of your slide, do not forget to write the names of your
