Introduction
Clinical pathway is the heart of quality and
patient safety. It is of major importance to
guide multidisciplinary teams- including
.clinicians, managers and patients
Many hospitals, managed care facilities and
home health services nationwide use clinical
pathways or care mapping to coordinate care
for a caseload of patients
Clinical Pathway
Clinical pathways as a way to
analyze costs, reduce variation,
.and Improve clinical quality
Aim/ purpose of Clinical pathways
Enhance the quality of care across the continuum of care.
Improving patient outcomes by providing a mechanism to coordinate
care.
Promoting patient safety.
Increasing patient satisfaction, and integration of knowledge, skills,
and experience of the nurse.
Optimize outcomes of the patient, family, nurse, and hospital
system.
Standardize important aspects of care.
Reduce unnecessary delays in care.
Reduce costs
Elements of clinical pathway
Patient Population: Covering patient groups in the treatment
and recovery is relatively predictable.
Time Frames: The identified time frame may be minutes,
hours, days, weeks, or phases. Conditions requiring emergency
treatment (myocardial infarction, head injury, stroke) might be
divided into 15-minute intervals.
Interventional Categories: Interventional categories consist of
the groups of activities that make up a comprehensive treatment
plan as medications, blood tests, procedures, and Consultations.
Outcomes: Defined outcomes provide the focus for patient care
activities. All well-written pathways include outcomes identified
for each time interval
Variance Record: Variance is defined as any deviation from the
pathway
Phases of Clinical pathways
Evaluate current practices.
Choose a clinical condition
Define the guidelines that must be incorporated
into the pathway
Map the care pathway.
Define the target outcomes.
Engage your team.
Implement and evaluate the efficiency and
effectiveness of the care pathway.
Barriers and Facilitators to
Implementation
Barriers Facilitators
- Lack staff involvement Clinical and management -
Active resistance due to - staff involvement
lack of involvement Staff training Allow for -
Lack awareness Time - flexibility
constraint Insufficient staff Process and outcome -
.Increased cost measures Provide
-Poor imbursement - • reimbursement
Efficient supply and utilization of personal
protective equipment
Efficient supply and utilization of ICU
devices, consumables, and pharmaceuticals
More effective ICU triage
Better end-of-life care
Reimagining communication with families
